Can serum white birch (Betula verrucosa) pollen antigen (Bet v 1) immunoglobulin E measurement distinguish between sensitization and allergy?

Abstract: Serum Bet v 1 IgE measurement may be able to differentiate sensitization from allergy in subjects with sensitization to Betulaceae pollen allergens. Thus, Bet v 1 IgE measurement should be considered a useful step in the management of patients with allergic rhinitis due to Betulaceae pollen allergens.

“…Possible explanations may consist in a particular botanic milieu where Betulaceae predominates. This is in agreement with the high prevalence of birch allergy with a Bet v 1 polarized pattern (18). This Bet v 1 preponderance influences the low prevalence of oral allergy syndrome in this region (19).…”

“…As all Bet v 6 sensitizations remain clinically silent, this finding contradicts recently published data that the level of specific IgE is the most important predictor for allergic symptoms [13]. As incidental finding, our data confirm the recent study reporting that patients with clinical relevant sensitization show higher specific IgE levels for the major allergen Bet v 1 than patients with silent sensitization [14]. …”

“…Furthermore, all patients underwent NPT to distinguish between sensitization and allergy, even if the clinical history was suggestive of either the presence or the absence of allergic symptoms under exposure to HDM. Many other studies have the limitation that the clinical relevance of sensitization is not always proven by provocation tests, but only partially assumed based on data from the clinical history [10,15,23]. Rhinitis can by caused by many different factors.…”

“…Therefore, health professionals try to reduce the need for challenge tests in food allergy. In recent years, the question of whether in vivo challenges might be replaced by less risky in vitro tests is increasingly debated with regard to airborne allergens [10].…”
