Can pelvic tilting be ignored in total hip arthroplasty?

Abstract: INTRODUCTIONThe orientation of acetabular component is influenced by pelvic tilt, body position and individual variation in pelvic parameters. Most post-operative adverse events may be attributed to malposition of the component in the functional position. There is evidence that orientation of the pelvis changes from the supine to standing position. Authors report a case of recurrent dislocation after total hip arthroplasty due to excessive pelvic tilting.PRESENTATION OF CASE A 69-year old female with coxarthro… Show more

“…Position of the pelvis is a key in determining the sagittal balance of the spine. It should be remembered that most of the compensatory abilities in imbalance are possible due to a change in the position of the pelvis [14,22,[49][50][51][52][53]. The main parameter of the pelvis is PI (deviation of the pelvis from the vertical axis), due to the fact that it is an unchangeable morphological and individual parameter for each person.…”

“…Dynamic parameters are interrelated, with the strongest correlation among healthy people being traced between lumbar lordosis GLL and the sacrum slope SS (p < 0.001; R = 0.86) [14,17,54,55]. The ideal value of RT (pelvic tilt) does not exceed 50 % of the PI parameter, and the ideal value of the sacrum slope SS exceeds 50 % of PI [55][56][57][58].…”

“…considered that if the acetabular anteversion is ≥20° in the supine position, then the hip‐joint prosthesis is relatively stable in the sitting position. Shon et al . reported an individual case who developed dislocation after THA.…”

“…Therefore, the question whether PT angles measured in the supine position are adequate for acetabular cup alignment without an adjustment or without considering individual patient differences is highly relevant [ 10 ]. The accurate knowledge of the spatial orientation of the pelvis in an upright position is extremely important during acetabular cup implantation (mostly performed in supine position), because the pelvis position influences the final hip cup inclination and anteversion angles [ 6 , 11 , 12 ]. The inclination and anteversion angles have been advocated to influence the outcomes and survival rates in THA [ 13 ].…”
