Our aim is to investigate the correlation between the mean platelet volume (MPV) levels and platelet counts of the septic children with 28-day mortality risk. MPV at admission (MPV adm ), MPV at 72 nd hour (MPV 72h ) and the difference between these two parameters (ΔMPV 72h-adm ) and platelet counts were recorded retrospectively. The control group consisted of 100 healthy children matched for age, gender, and ethnicity. One hundred eighty six children were enrolled into the study. The study group had significant higher MPV values than those of control group. There were 156 survivors and 32 nonsurvivors in study group with a mortality rate of 17%. Nonsurvivors had significantly higher MPV adm (p: 0.001), MPV 72h (p: 0.001), ΔMPV 72h-adm (p < 0.001) and lower platelet count (p: 0.002) than survivors. MPVadm (OR 2.39), MPV 72h (OR 4.23), ΔMPV 72h-adm (OR 6.4), platelet count (OR 7.3), and need for mechanical ventilation support (OR 9.76) had significant effect on 28-day mortality risk at logistic regression analysis. At the receiver operating characteristic analysis (ROC) the cutoff values for MPV adm , MPV 72h , and ΔMPV 72h-adm were found to be 9 fL, 9.86 fL, and 0.79, respectively. Kaplan-Meier analysis and log-rank test proved that these cutoff values were significantly associated with the time of survival. Septic children who had high MPV levels at admission and whose MPV levels increased during follow up had higher risk of mortality. With the results of further researches targetting large groups of pediatric patients, MPV adm , MPV 72h , and ΔMPV 72h-adm values can be fast and reliable markers for early diagnosis of sepsis and mortality prediction.

Aim: Levels of Serum Procalcitonin (PCT), C-Reactive Protein (CRP), Leukocyte (WBC) and Mean Platelet Volume (MPV) were evaluated in sepsis patients. We evaluated the diagnostic accuracy of different inflammatory markers to discriminate sepsis caused by different pathogens. Material and Method: In this study we included 126 episodes of bacteremia from 126 patients with sepsis. Medical records of patients who had bacteremia caused by Gram-negative bacteria (GN), Gram-positive bacteria (GP) or yeast were reviewed, and information about PCT and other inflammatory markers was recorded. Mann-Whitney U test and Spearman's correlation analysis were used for statistical analysis. Results: 60.4% of the cases (n=76) were GN, 31.7% (n=40) were GP and 7.9% (n=10) were yeast. There was a statistically significant difference between PCT, CRP, and MPV levels according to the microorganism (p=0.001, p<0.01). There was no statistically significant difference between WBC measurements according to the microorganism (p>0.05). Discussion: According to the microorganism, gram positivity cutoff points were determined for PCT as (≤12.17), CRP as (≤9.70) and MPV as (≤9.32). In bilateral comparisons, there was a significant increase in GN and yeast sepsis, while a significantly low level was found in GP sepsis. There was no significant difference between GN and yeast. When the measurements of PCT according to bacterial species are examined, the highest measurement was 44.6 ng/ml with Escherichia coli bacteria, followed by Candida bacteria with 12.6 ng/ml.

ÖZObjective: In recent years, a lot of non-invasive tests have been examined for estimating the severity of liver fibrosis in patients with chronic hepatitis B (CHB). We aimed to evaluate the role of simple and valuable platelet-derived indices in estimating the stage of fibrosis and cirrhosis in patients with CHB. Materials and Methods:A total of 434 biopsy-proven naive CHB patients were included in the study. Liver biopsy samples were examined using the ISHAK scoring system. Age, sex, complete blood count parameters and the stage of fibrosis of the patients were recorded. The patients were evaluated according to the severity of fibrosis (stage 0-2: No fibrosis/mild fibrosis and stage 3-6: Significant fibrosis) and the presence of cirrhosis (stage 0-4: No cirrhosis, stage 5-6: Cirrhosis). Results: Of the 434 patients included in the study, 252 (58.1%) were men and 182 (41.9%) were women. The mean age of the patients was 42.7±12.2 years. Significant liver fibrosis was found in 108 (24.9%) patients and cirrhosis in 43 (9.9%) patients. In dual analysis, alterations in many platelet-derived parameters were observed. Receiver operating characteristic curve analysis revealed that red blood cell distribution width to platelet ratio index was the highest area under the curve value (0.777) in predicting significant liver fibrosis and the cutoff value was 0.0814, sensitivity was 56% and specificity was 87%. Conclusion: Platelet-derived indices may play an important role in observing the progression of liver fibrosis and cirrhosis. Platelet ratio index is significantly increased in advanced liver fibrosis in patients with CHB, and can be defined as independent predicting factor in liver fibrosis.
