Calcium supplementation and ototoxicity in patients receiving cisplatin

Abstract: We have studied the effect on ototoxicity of maintaining serum calcium concentration by calcium gluconate infusion in cancer patients receiving highdose cisplatin in a randomized study in two groups: 11 patients received calcium gluconate, 4 mg kg−1 i.v. infusion during cisplatin therapy; 11 other patients without any calcium supplementation served as controls. All of them received the first course of chemotherapy, based on cisplatin, 120 mg m2 with a hydration schedule. An audiogram was performed in each pati… Show more

“…However, most of those studies were performed in children and a Cochrane systematic review concluded that the evidence was insufficient to allow for recommendation of such preventative treatments [24] . Similarly, our group has previously studied the effect on ototoxicity of maintaining serum calcium concentrations via calcium gluconate infusion in HNC patients treated with highdose cisplatin, but we did not observe any protection against ototoxicity using this treatment [25] . Therefore in our opinion, while novel preventative treatments should continue to be sought, in the meanwhile, switching to less ototoxic treatment regimens, such as carboplatin or cetuximab, is a useful option.…”

“…Although not statistically significant, the combined cisplatinclotrimazole treatment of rats with 9L gliomas resulted in an additional survival benefit over the survival of rats treated with either drug alone. 17 When released from intracellular stores, but not moved from extracellular space, Ca ϩϩ plays a role in cisplatin-induced cell injury. The results of in vitro experiments do not always correspond to results that are obtained in vivo.…”

“…Mucositis is a dose-limiting toxicity in the first regimen and pulmonary toxicity in the second. Carboplatin has been used for selected patients due to its improved toxic profile compared with cisplatin especially as regards nephrotoxicity and ototoxicity [9]. Clinical series in general combine results of cancer in the oral cavity with cancer of the larynx or hypopharynx.…”
