Multidisciplinary Approach in Advanced Cancer of the Oral Cavity: Outcome with Neoadjuvant Chemotherapy according to Intention-to-Treat Local Therapy

Abstract: Objectives: To determine outcomes in local-regional control and overall survival in patients with squamous locally advanced cancer of the oral cavity, based on intention-to-treat with neoadjuvant chemotherapy followed by surgery or radiation therapy. Methods: Two hundred and four out of 1,089 patients analyzed met the defined criteria. All had squamous cell carcinomas of the oral cavity in stage III or in nonmetastatic stage IV and were selected for surgery or radiation therapy (if located in the tonsils or in… Show more

“…Multivariate analysis showed that nuclear localization of ANXA1 in OSCC, regardless of tumor STNM status, could be used to determine patient prognosis. In this study, we were unable to include some important pathologic (e.g., margin status, lymph node extracapsular extension) and treatment (e.g., extent of surgery, application of adjuvant radiations) variables that could affect patient's survival [31] in the multivariate analysis because those data were missing for some patients. Further studies using larger numbers of cases are needed to investigate whether ANXA1 nuclear expression is an independent marker of poor survival after adjustment for other important pathologic and treatment variables in patients with oral SCC.…”

“…This survival improvement had been maintained when the response to chemotherapy was combined with other prognostic factors in the multimultivariate analysis, suggesting that response was a powerful, individualized prognostic factor [15]. The reports by the Department of Veteran Affairs Laryngeal Study Group [16] and the European Organization [17] for Research and Treatment of Cancer demonstrated IC with PF followed by radiation therapy in responders with advanced laryngeal, and hypopharyngeal cancer, respectively, resulted in two thirds of the patients retaining their larynx without a survival disadvantage compared with non-responders who received surgery and post-operative radiation therapy.…”
