Orbital cystic lesions encompass an array of congenital and acquired conditions with diverse aetiologies, ranging from benign, asymptomatic cysts to sight-threatening infectious or malignant disease. 1,2 They account for 14% to 21% of all orbital lesions. 2 Macroscopically, cystic lesions appear as saccular, fluid-filled structures encapsulated by a clearly demarcated wall. 1 They are commonly classified according to the cellular origin or etiology. 3,4 In a 2004 review of childhood orbital lesions, Shields and Shields 5 categorized cystic lesions by cellular origin (surface epithelium-derived, teratomatous, neural, secondary, infectious, and noncystic lesions with a cystic component). Another classification system, proposed by Kaufman et al, 6 in 1998, organizes cystic lesions by etiology; that is, congenital, acquired, or secondary cysts arising from adjacent structures. This review will expand upon the latter scheme, employing the following classifications; congenital (epithelial and nonepithelial), vascular, infectious and inflammatory, neoplastic, and miscellaneous. True cysts and cyst-like lesions, that is, those which appear cystic on imaging, will be covered, with a particular focus on the radiologic characteristics seen.Congenital lesions, such as dermoid cysts and orbital teratoma, generally present in infancy or early childhood and are usually benign; those which present later in life are more likely acquired, although lesions such as deep orbital dermoids may present in adulthood. Background Orbital hydatid disease, while rare, should be included in the differential diagnosis of unilateral proptosis, particularly in endemic areas. Accurate diagnosis and comprehensive management are essential for effective treatment and favorable long-term outcomes. Case Presentation A 12-year-old boy presented with a one-month history of diplopia and left-sided proptosis. A CT scan revealed a cystic mass within the left medial rectus muscle. Surgical intervention confirmed the presence of a hydatid cyst following cyst rupture and irrigation with hypertonic saline. The patient underwent a two-month course of albendazole therapy. Initially, the patient experienced persistent exotropia and diplopia, but at the three-year follow-up, he exhibited no diplopia or proptosis and only mild residual exotropia. Conclusion This case underscores the importance of considering orbital hydatid cysts in the differential diagnosis of unilateral proptosis in endemic regions. Heightened awareness, accurate diagnosis, and a tailored therapeutic approach, including surgical removal and antiparasitic treatment, are crucial for successful management and improved long-term outcomes.

Background: Hydatid disease is a tapeworm parasitic infection caused by Echinococcus granulosus that commonly affects the liver and lungs during its life cycle in the human body. Orbital involvement is a very rare occurrence and isolated orbital Echinococcosis is extremely rare. Case Presentation: A 60-year-old female Eritrean woman presented with 3-month history of painless protrusion of the left eye. Physical examination showed significantly decreased visual acuity of the left eye, left eye proptosis, and papilledema. Laboratory investigations were normal and orbital CT scan examination was suggestive of orbital hydatid cyst. After giving two weeks course of oral Albendazole, anterior orbitotomy and cyst excision was done. Post-op biopsy was conclusive of orbital hydatid cyst. She had mild lid swelling and adduction deficit on her post-operative follow-up. Subsequent longer duration of follow-up of the patient was not possible since the patient relocated back to Eritrea. Conclusion: Orbital hydatid cyst is an extremely rare presentation of human Echinococcus granulosus infection. However, it should be considered one of the differential diagnoses among patients living in an endemic area and presenting with proptosis of the eye. A preoperative course of anthelmintic followed by surgical excision of the cyst is the definitive management.
