C-reactive protein, white blood cells, and neutrophils as early predictors of postoperative complications in patients undergoing laparoscopic sleeve gastrectomy

Abstract: Because CRP detected leak or abscess after LSG with remarkably higher sensitivity and specificity than WBC or NEU, CRP seems to be a more accurate market for the early detection of these complications.

“…It is said to be the organism’s response to trauma. According to the literature the CRP level increased in all patients within the first days after bariatric surgery, reaching the highest level on the 2 nd , 3 rd or 5 th POD, and then decreases and normalizes within the first month after surgery [1315]. In this study the CRP level on the 2 nd POD was almost 2 times higher among patients with non-septic complications and 4 times higher among patients with septic complications, in contrast to the remainder (Figure 2).…”

“…Mean postoperative CRP levels were 61.3 mg/l in uncomplicated patients and 161.3 mg/l in patients with complications (p = 0.0233). What seems to be important is the kinetics of CRP values after bariatric surgery [12,13]. After laparoscopic sleeve gastrectomy, it has been shown that CRP values are increased preoperatively and in the postoperative course up to 1 year after procedure [14].…”

“…Thus, the diagnosis of fistula and/or abscess is not easy, which is why we evaluated whether the measurement of C-reactive protein (CRP) [9] and procalcitonin (PCT) could be of help in the detection of gastric fistula after sleeve gastrectomy. A prospective study of 177 patients was realized by Albanopoulos K et al he mentioned that on day 3, the sensitivity and specificity of CRP reached 100 %, and on day 5, CRP achieved 83.2 % sensitivity and 100 % specificity [9]. Discovered in the 1930s, CRP belongs to the family of proteins involved in the acute phase of inflammation (acute phase proteins).…”

“…Some authors define the third and fourth day after surgery as a cutoff point [21,22,24]. Other authors could show no predictive value for an early diagnosis of bowel leak or inflamed disease [19,20,23,25]. …”

“…According to Riss et al [18], preoperatively increased CRP values in patients with Crohn's disease seem to be associated with a negative outcome. Most studies focus on the forecast of serious infections or leakage after intestinal surgery [19,20,21,22,23,24,25]. Some authors define the third and fourth day after surgery as a cutoff point [21,22,24].…”
