Poststroke fatigue (PSF) is reported to occur in 30%–72% of all patients with stroke. PSF not only is a symptom of stroke but has also been reported to adversely affect the prognosis of patients with stroke. However, no treatment has had a significant effect on PSF. In East Asian countries, several herbal medicines have been used to treat stroke, with Buyang Huanwu Tang (BHT) being the most common. This review aimed to evaluate the efficacy and safety of BHT for PSF. A literature search was conducted on MEDLINE, CENTRAL, Scopus, CiNii, CNKI, OASIS, NDSL, and KTKP databases for randomized controlled trials that evaluated the effects and safety of BHT on PSF. Six studies (n = 427) were included. The overall methodological quality of these studies was relatively low. In the adjunctive BHT group, the meta-analysis indicated statistically significant improvements in the Fatigue Severity Scale score (mean difference −1.49, 95% CI [−2.25, −0.73]) and total clinical efficacy rate (risk ratio 0.11, 95% CI [0.03, 0.41]) compared to those in the nonherbal group. Adverse events were only reported in one study, and no serious adverse events occurred. BHT administration might be effective in the treatment of PSF. We were unable to draw definitive conclusions owing to the limitations of the included studies. The trial is registered with CRD42019130178 in PROSPERO.

Stroke merupakan salah satu penyebab utama kematian dan disabilitas di seluruh dunia. Salah satu komplikasi stroke adalah fatigue post stroke (FPS), dimana prevalensi berkisar 25%-85%. Mekanisme FPS belum diketahui secara pasti kemungkinan dapat disebabkan oleh fatigue fisik dan mental. Faktor-faktor yang menyebabkan FPS antara lain disabilitas fisik dan gangguan psikologi. Intervensi baik fisik maupun non fisik belum terbukti secara efektif mengatasi FPS. Tujuan dari studi ini untuk melihat keefektifan intervensi yang bersifat non fisik dalam mengatasi FPS. Studi ini merupakan literature review dengan pencarian artikel menggunakan database Scopus, Ebsco, dan Science Direct dengan kata kunci fatigue post stroke, non-physical intervention, dan stroke management. Kriteria inklusi adalah jurnal full text dalam Bahasa inggris, intervensi pada manusia, penelitian Randomize Control Trial (RCT) atau Quasi experiment, jurnal yang terbit dari tahun 2015-2020 dan penelitian yang mengukur tingkat FPS sebagai tujuan utamanya. Hasil studi literatur didapatkan lima artikel RCT yang sesuai kriteria inklusi, dari kelima artikel didapatkan tiga jenis intervensi yaitu obat psikostimulan, Traditional Chinese Medicine (TCM) dan intervensi psikologi. Meskipun tidak semua intervensi menunjukkan hasil yang signifikan, intervensi dengan modafinil, Cognitive Behavioral Therapy dan TCM dapat menurunkan FPS. Hasil dari telaah literatur dapat dijadikan sebagai bukti dasar dalam penanganan fatigue pada pasien post stroke. Kata-kata kunci: fatigue post stroke, intervensi non fisik, manajemen stroke ABSTRACT Stroke is one of the leading causes of death and disability worldwide. One of the complications of stroke is fatigue post stroke (FPS), where the prevalence ranges from 25% -85%. The mechanism of FPS is not established yet, it might be caused by physical and mental fatigue. Factors that causes FPS include physical disability and psychological disorders. Both physical and non-physical interventions have not been proven to be effective in treating FPS. The purpose of this study is to explore the effectiveness of nonphysical interventions in overcoming FPS. Literature review was used in this study by searching articles using the 3 database, namely Scopus, Ebsco, and Science Direct with the keywords fatigue post stroke, nonphysical intervention, and stroke management. Inclusion criteria are full text journals in English, human intervention, Randomize Control Trial (RCT) or Quasi experiment research, journals published from 2015-2020 and studies that measure FPS levels as their main objectives. The results of the literature study found five RCT articles that fit the inclusion criteria. Three types of intervention were obtained in these articles, there were psychostimulant drugs, Traditional Chinese Medicine (TCM) and psychology intervention. To explore the Radix Paeoniae Rubra-Flos Carthami herb pair's (RPR-FC) potential mechanism in treating ischemic stroke (IS) by network pharmacology and molecular docking technology. The Traditional Chinese Medicine Systems Pharmacology Database was used to screen the active components of the RPR-FC, and Cytoscape 3.8 software was used to construct a network map of its active components and targets of action. The GeneCards and OMIM databases were used to identify disease targets of IS, and the common targets were chosen as research targets and imported into the STRING database to construct a protein–protein interaction network map of these targets. R language software was used to analyze the enrichment of GO terms and KEGG pathways, and explore the mechanisms of these targets. Molecular docking technology was used to verify that the RPR-FC components had a good bonding activity with their potential targets. A total of 44 active components, which corresponded to 197 targets, were identified in the RPR-FC. There were 139 common targets between the herb pair and IS. GO functional enrichment analysis revealed 2253 biological process entries, 72 cellular components entries, and 183 molecular functions entries. KEGG pathway enrichment analysis was mainly related to the NF-kappa B signaling pathway, the TNF signaling pathway, apoptosis, the MAPK signaling pathway, the PI3K-Akt signaling pathway, the VEGF signaling pathway, etc. The molecular docking results showed the components that docked well with key targets were quercetin, luteolin, kaempferol, and baicalein. The active components (quercetin, luteolin, kaempferol, and baicalein) of the RPR-FC and their targets act on proteins such as MAPK1, AKT1, VEGFA, and CASP3, which are closely related to IS. 1 These targets are closely related to the NF-kappa B signaling pathway, the MAPK signaling pathway, the PI3K-Akt signaling pathway, the VEGF signaling pathway, and other signaling pathways. These pathways are involved in the recovery of nerve function, angiogenesis, and neuronal apoptosis and the regulation of inflammatory factors, which may have a therapeutic effect on IS.
