“Burned out” testicular seminoma presenting as a primary gastric malignancy

Abstract: In contrast to primary gastric adenocarcinomas, germ cell tumors are potentially curable even when metastatic. It is therefore essential for clinicians and pathologists to be aware of the spectrum of unusual manifestations of germ cell malignancies. Here we report on a 55-year-old man who presented with clinical and endoscopic features indicative of a primary gastric carcinoma. Surprisingly, the ulcerative mucosal lesion was found to be due to a metastasis from an occult, "burned-out" testicular seminoma. This… Show more

“…Testis tumor markers that are within the normal range could not be relied on to establish the diagnosis. To our knowledge, this is the largest series of this rare entity, as the literature only contains 25 instances that have been reported either as case reports or as small cohorts . Only one of these cases involved nonmetastatic disease .…”

“…The differential diagnosis of 18F-FDG uptake in the stomach includes more common benign entities such as physiological uptake [12]; gastritis (with or without H. pylori involvement) [13]; and malignant entities. These malignant entities include gastric adenocarcinoma [14], signet ring cell carcinoma [15], diffuse large B-cell lymphoma [16], MALT lymphoma [17], primary mediastinal B-cell lymphoma [18], neuroendocrine tumour [19], gastrointestinal stromal tumour (GIST) [20] and metastases [21]. …”

“…In the clinical setting of patients with 'burned-out' germ cell tumors, metastatic lesions are first identified in the presence of regressed primary tumors, the latter diagnosed by a distinct histological appearance [7,8]. However, our case report differs in demonstrating a clear aggressive behavior for the primary tumor upon clinical manifestation post-metastatectomy, with radiological growth from undetectable to an 11 cm lesion over four months, which is inconsistent with a 'burned-out' primary.…”
