This cancer is the cause of deaths in high-income countries and the second cause of death in low and middle income countries (Payandeh et al., 2015b). BC is the most common cause of death in middle-aged women AbstractBreast cancer (BC) is the most common cancer in females (27% of the total) and the main cause of death (16%) due to cancer in women in developed and developing countries. Variations in its incidence rate among geographical areas are due to various contributing factors. Since there have been a lack of studies on this topic in our country, the present spatial analysis of breast cancer incidence in Iran in 2009 was conducted using data from the national cancer registry system. The reported incidences of the disease were standardized according to the World Health Organization population and the direct method. Then data was inserted into the GIS software and finally, using the Hot Spot Analysis (Geties-Ord Gi), high-risk areas were drawn. Provinces with incidences 1.96 SD higher or lower than the national average were considered as hot spots or cold spots, at the significance level of 0.05%. In 2009, a total of 7,582 cases of BC occurred in Iran. The annual incidence was 33.2 per hundred thousand people. Our study showed that the highest incidence of BC in women occurred in the central provinces of the country, Tehran, Isfahan, Yazd, Markazi and Fars. The results of hot spots analysis showed that the distribution of high-risk BC was focused in central parts of Iran, especially Isfahan province (p <0.01). The other provinces were not significantly different from the national average. The higher incidence in central provinces may be due to greater exposure to carcinogens in urban areas, a Western lifestyle and high prevalence of other risk factors. Further epidemiological studies about the etiology and early detection of BC are essential.

Purpose: To make a comparison of single voxel magnetic resonance spectroscopy (SV-MRS) and dynamic contrast enhancement (DCE) MRI for differentiation of benign and malignant breast lesions in a sample of Iranian women. Materials and Methods: A total of 30 women with abnormal breast lesions detected in mammography, ultrasound, or clinical breast exam were examined with DCE and SV-MRS. tCho (total choline) resonance in MRS spectra was qualitatively evaluated and detection of a visible tCho peak at 3.2 ppm was defined as a positive finding for malignancy. Different types of DCE curves were persistent (type 1), plateau (type 2), and washout (type 3). At first, lesions were classified according to choline findings and types of DCE curve, finally being compared to pathological results as the standard reference. Results: this study included 19 patients with malignant lesions and 11 patients with benign ones. While 63.6 % of benign lesions (7 of 11) showed type 1 DCE curves and 36.4% (4 of 11) showed type 2, 57.9% (11of 19) of malignant lesions were type 3 and 42.1% (8 of 19) type 2. Choline peaks were detected in 18 of 19 malignant lesions and in 3 of 11 benign counterparts. 1 malignant and 8 benign cases did not show any visible resonance at 3.2 ppm so SV-MRS featured 94.7% sensitivity, 72.7 % specificity and 86.7% accuracy.Conclusions: The present findings indicate that a combined approach using MRS and DCE MRI can improve the specificity of MRI for differentiation of benign and malignant breast lesions.

A Survey in the Basal Like Breast Carcinoma Phenotype in North East of Iran

Background: Breast carcinoma is not a homogenous disease. It is divided into 5 pathological subtypes of luminal1, luminal2, strongly Her2 positive, basal like, and normal breast like. Basal like breast carcinoma that accounts for about 15% of all breast cancers has an aggressive clinical behavior with the features of high nuclear grade, negative estrogen receptor, progesterone receptor, and Her2 reactivity (triple negative). Objectives: We aimed at identifying the prevalence of basaloid phenotype among triple negative (TN) cases in our region via immunohistochemistry (IHC) staining using basaloid markers. Methods: We reviewed breast cancer patients in Omid and Imam Reza hospitals, Mashhad, Iran, between 2003 and 2007. We obtained the paraffin blocks from TN cases for immune-staining using cytokeratin 5/6 (CK5/6), cytokeratin 14 (CK14) and epidermal growth factor receptor1 (EGFR1) markers. Results: The incidence of TN disease among breast cancers was 21% (156/747). Based on IHC reactivity with at least one of the basaloid markers, from 59 available samples, 44 (75.4%) were basaloid. Conclusions: In our region, most triple negative tumors were basal like breast cancer (BLBC). Among these cases, most immunereactivity was observed for EGFR1, followed by CK14 and CK5/6.
