Breast Implant–Associated Anaplastic Large Cell Lymphoma in Australia and New Zealand: High-Surface-Area Textured Implants Are Associated with Increased Risk

Abstract: This study has calculated implant-specific risk of breast implant-associated ALCL. Higher-surface-area textured implants have been shown to significantly increase the risk of breast implant-associated ALCL in Australia and New Zealand. The authors present a unifying hypothesis to explain these observations.

“…Management of refractory or recurrent disease is also not standardized and should be individualized. There have been reports that radiation or chemotherapy for local recurrence has been successful (de Boer et al, 2018;Loch-Wilkinson et al, 2017). Since patients with advanced presentation at initial diagnosis are treated as systemic ALCL, relapsed disease should be as well.…”

“…The American Society of Plastic Surgeons reports that there are 673 worldwide cases of BIA-ALCL as of January 16, 2019 (ASPS, 2019), with 16 disease-related deaths (ASPS, 2018). Incidence estimates are highly variable but range from roughly 1:3,000 to 1:30,000 patients with textured implants (Clemens et al, 2017;de Boer et al, 2018;Loch-Wilkinson et al, 2017). While the majority of cases have been reported around textured implants, case reports exist in women with smooth implants, and much of the data in MDR reports is incomplete.…”

“…BIA-ALCLs have been reported in association with all types of textured implants. However, studies suggest that some manufacturers may present a higher risk than others 6 . No case has been confirmed in patients who have used only smooth implants.…”
