Background: Identifying risk factors of breast cancer is a key point for preventive strategies to reduce the incidence. The aim of current study was to determine most important risk factors for breast cancer in the Eastern Mediterranean Region (EMR) using a systematic review. Materials and Methods: PubMed, Scopus, Web of Science till August 24, 2012 and the reference lists of all included studies were searched. Analytic studies which had reported odds ratios (OR), relative risk (RR) or required data to calculate them were included. A total of 343 studies were critically appraised and finally 30 studies were meta-analyzed. Heterogeneity between the studies was assessed by I² and Cochran's Q. Egger's test was used to assess publication bias. Results: Twenty five casecontrol studies, one nested case-control and four cohort studies were included. The largest ORs were obtained for history of no live birth (2.25; 95%CI: 1.58-3.18), body mass index (BMI) more than 30 (2.21; 95%CI: 1.71-2.36), age at first pregnancy more than 30 years old (1.52; 95%CI: 1.30-1.77) and meat consumption more than three times per week (1.39; 95%CI: 1.03-1.87). The other important predictors were higher education and smoking as risk factors, physical activity and ovulatory stimulating medication as protective factors. Conclusions: The most important predictors of breast cancer in EMR were history of no live birth, BMI more than 30, age at first pregnancy more than 30 years old, physical inactivity and smoking. Almost all these risk factors are consistent with known risk factors for this cancer in other parts of the world.

Breast cancer primary prevention is a high research priority due to the high psychological and economic costs. The disease is a multistep process and several risk factors have been recognized. Over the past three decades numerous studies have investigated the association of lifestyle with breast cancer, showing independent effects of various factors. We report here a summary of the present state of knowledge on the role of lifestyle patterns, such as physical activity, diet, smoking, hormone therapy, and experience of psychological stress in the modulation of breast cancer in women, and discuss commonly accepted biological mechanisms hypothesized as responsible for the associations. The findings indicate that regular physical activity of moderate to vigorous intensity is probably linked with the decreased breast cancer risk among postmenopausal females and suggestive for a decrease of the risk in premenopausal women. In contrast, the consumption of high-fat diet, alcohol intake, and use of combined estrogen and synthetic progestagen hormonal therapy may increase the risk. Epidemiological findings dealing with a role of smoking and experience of psychological stress are conflicting.

RESUMENEl sentido de coherencia conduce a evaluar las circunstancias de la vida como significativas, predecibles y manejables por lo que está considerando como una fuente que reduce el estrés y promociona la vida saludable. Por otro lado, las múltiples demandas que se plantean a los estudiantes en su periodo universitario pueden desencadenar estrés. Con este estudio hemos querido averiguar si el sentido de coherencia está relacionado con el cansancio emocional. Los participantes fueron 960 estudiantes universitarios (muestreo accidental), siendo el 81% mujeres. Respondieron a dos cuestionarios de autoinforme (el Cuestionario de Sentido de Coherencia SOC-13 y la Escala de Cansancio Emocional ECE). Se realizaron análisis de correlación y la prueba t de Student para averiguar las diferencias entre grupos. Las mujeres puntúan más alto que los hombres en cansancio emocional y los alumnos de tercero presentan mayores niveles de sentido de coherencia que los de primer curso. Los estudiantes con niveles altos de sentido de coherencia poseen niveles bajos de cansancio emocional, por lo que sería conveniente diseñar y poner en marcha estrategias para aumentar el sentido de coherencia disminuyendo así el cansancio emocional.ABSTRACTThe sense of coherence helps us view life events as significant, predictable, and manageable and it is thus thought to reduce stress and promote health. In contrast, the multiple demands faced by university students can trigger stress. In this study, we aimed to determine if the sense of coherence was related to emotional exhaustion. Participants were 960 university students (convenience sampling), 81% were women. They completed two self-report questionnaires: the SOC-13 Sense of Coherence Questionnaire and the Emotional Exhaustion Scale. We did correlation analyses and the Student’s t test to detect differences between groups. Women scored higher than men in emotional exhaustion. The third year students achieved higher scores in sense of coherence than first year students. Students with high scores in sense of coherence showed low levels of emotional exhaustion. These results indicate that it would be useful to design and implement strategies that help increase the sense of coherence and therefore reduce emotional exhaustion in university students.
