Ö ÖZ ZE ET T A Am ma aç ç: : Dokuz Eylül Üniversitesi Tıp Fakültesi Anestezi Yoğun Bakım (AYB)'da tanı almış erişkin beyin ölümü (BÖ) olgularının retrospektif incelemesini yapmak hedeflendi. G Ge er re eç ç v ve e Y Yö ön n--t te em ml le er r: : Eylül 2004-Aralık 2011 tarihleri arasında AYB'de BÖ tanısı almış 93 hastanın kayıtları retrospektif olarak incelendi. Arşiv kayıtları eksik olan 29 hasta ve 18 yaşından küçük 4 hasta çalışma dışı bırakıldı. Yaş, cinsiyet, yatış tanısı, AYB'de kalış süresi, BÖ tanı günü, bağış oranı, yatış günü ve BÖ tanı günündeki rutin laboratuar verileri; BUN, Cre, Na + , K + , Cl -, Ca +2 , CK, AST, ALT, Tbil, Dbil, APTT, PT, INR, D-Dimer ve insülin-desmopressin-vazopresör/inotrop infüzyon durumları kaydedildi. Demografik değişkenler için tanımlayıcı istatistikler verilmiştir. Laboratuvar veri değerlerinin karşılaştırılmasında eşleştirilmiş t testi kullanıldı. p<0,05 istatistiksel olarak anlamlı kabul edildi. Background: Inflammatory events following brain death (BD) decrease the quality of donor organs which can affect the outcome of the transplantation. It experimentally and clinically is verified that the level of inflammatory cytokines is increased in the BD of the donor results. In experiments, the in vitro use of vitamin C can successfully decrease the levels of inflammatory cytokines. Objectives: The aim of this study was to investigate the influence of vitamin C administration on the inflammatory status of the graft in BD liver donors. Methods: This study was conducted at Nemazee Hospital's transplant center (Shiraz, Iran). In this interventional study, the BD liver donors (n = 40) were randomly divided into two groups. The control group received only the routine intensive care unit (ICU) considerations, and the intervention group was treated with Vitamin C before harvesting organ (100 mg/kg, initially followed by 100 mg/kg/6h until organ removal). Blood samples were taken from BD patients in the intervention and control groups 3 times: 6 hours before operation, immediately after laparotomy, and immediately prior to clamping the aorta to assess the gene expression ratio of Interleukin 1 beta (IL1B) and Interleukin 10 (IL10) cytokines using real time polymerase chain reaction (PCR) (n = 40). Soluble serum cytokines were measured by enzyme-linked immunosorbent assay (ELISA) (n = 24). Results: No significant differences were observed in mRNA expression ratio of IL1B and IL10 between two groups. Despite an acceptable decrease in serum concentration of the inflammatory markers IL1B and IL10 at time point 2 minus time point 1 (T2-1), no significant differences were observed in vitamin C-treated BD donors compared with the control donors. There was a significant difference among aspartate aminotransferase (AST) and alanine aminotransferase (ALT) changes on the 3rd to 1st days after operation between the control and intervention recipients (P < 0.05). Conclusions:The present study suggests a beneficial effect of vitamin C adsministration on post-transplant function of the liver from BD liver donors.

“…This report was similar to their report published in 2001, which found no clear reason for the increase, but implied the damage to the hepatocytes as a potential candidate. The study by Compagnon et al [35], published in 2002, observed that there are no significant changes in AST, ALT, LDH, gamma-glutamyl transpeptidase (GGT), and total bilirubin (except for alkaline phosphatase (AKP)) before organ removal in brain-dead donors compared with those that have been brain dead for 16 h. After orthotopic transplantation of donor livers cold-preserved for 24 h in the University of Wisconsin solution, there were no significant differences in AST, ALT, and LDH in two groups, which gradually became normal after 2-3 days. Golling et al [36] reported in 2003 that aspartate aminotransferase/glutamate-oxaloacetate transaminase (AST/GOT) of hypotensive and normotensive brain-dead pigs both increased after induction of brain death.…”

Brain-dead donors have become one of the main sources of organs for transplantation in Western countries. The quality of donor organs is closely related to the outcome of the transplantation. Experimental studies have confirmed the inferior graft survival of livers from brain-dead donors compared with those from living donors. Studies conducted in the past 10 years have shown that brain death is associated with effects on the decreased donor organ quality. However, whether the decrease in the viability of donor organs is caused by brain death or by the events before and after brain death remains uncertain. The purpose of this review is to introduce the advances and controversies regarding the influence of brain death on the viability of donor livers and to summarize the mechanisms of the different protective interventions for donor livers.
