Bone mineral density at extremely low weight in patients with anorexia nervosa

Abstract: Anorexia nervosa (AN) is a severe eating disorder characterized by an emotional and cognitive inability to maintain a normal weight.Quantitative and qualitative restriction of nutritional intakes, purging behaviours, physical hyperactivity and laxative abuse are the main strategies to control weight and shape. These behaviours are maintained by a persisting distortion of body image, generating a vicious circle of weight loss. 1 Prevalence varies between 0.9% and 4% among women and is 10 to 13 times lower among… Show more

“…Similar to duration of illness, participants included in groups indicated as manifesting a severe and enduring phenotype in case studies, clinical trials and studies, tended to have substantially lower BMIs than required per the inclusion criteria. For example, participants in the Bemer et al bone mineral density (BMD) study had a mean BMI of 12.60 ± 1.60 kg/m 2 , which was well below the < 16 kg/m 2 criteria [ 62 ].…”

“…This suggests that, after removing some confounding conditions, the SIRT1/sclerostin interplay remained crucial for bone homeostasis. Although BMD is still considered a major determinant of bone strength, fragility fractures are reported only from a limited percentage of patients affected by anorexia and low BMD [ 77 ]. This evidence, together with the fact that most individuals with a fragility fracture show BMD value in the osteopenic or even in the normal range [ 60 ], indicates that other factors participate in determining bone strength and fracture risk beyond BMD and TBS.…”

“…Secondary somatic complications to undernutrition, severe weight loss and low levels of micronutrients, in addition to heart problems (bradycardia) [ 17 ], hormonal problems (amenorrhea), intestinal problems or bone demineralisation, have also been studied [ 18 , 19 ]. Nevertheless, the loss of autonomy and reduction in muscle strength are not sufficiently described in the scientific literature [ 20 ].…”
