The emergence of severe acute respiratory syndrome corona virus 2 (SARS-CoV-2) at the end of 2019 in Hubei province China, is now the cause of a global pandemic present in over 150 countries. COVID-19 is a respiratory illness with most subjects presenting with fever, cough and shortness of breath. In a subset of patients, COVID-19 progresses to hypoxic respiratory failure and acute respiratory distress syndrome (ARDS), both of which are mediated by widespread inflammation and a dysregulated immune response. Mesenchymal stem cells (MSCs), multipotent stromal cells that mediate immunomodulation and regeneration, could be of potential benefit to a subset of COVID-19 subjects with acute respiratory failure. In this review, we discuss key features of the current COVID-19 outbreak, and the rationale for MSC-based therapy in this setting, as well as the limitations associated with this therapeutic approach.

Background Phosgene-induced lung injury is an important type of acute lung injury (ALI), which mainly leads to acute pulmonary edema. Currently, no effective clinical treatment has been developed yet. In the present study, the effects of lung-resident mesenchymal stem cells (LRMSCs), bone marrow mesenchymal stem cells (BMSCs) and human chorionic villi-derived MSCs (hCMSCs) were compared in phosgene-induced lung injury of male SD rats. Methods The changes in body weight, PaO2 and respiratory indexes were recorded. Rats were sacrificed at 6 h, 24 h, and 48 h. Expressions of TNF-α, IL-1β, IL-6, HGF and IL-10 were tested by ELISA. SP-C mRNA expression was assessed by RT-PCR. The MSCs migration was assessed using Transwell migration assay and Wound-healing assay. Hepatocyte growth factor (HGF) was added to the MSCs culture medium for 48 h. Expressions of p-GSK-3β and β-catenin were determined by Western blot. Results Significant improvements in body weight, PaO2 and respiratory indexes were observed after treatment with MSCs. BMSCs produced the highest effects followed by hCMSCs and LRMSCs. Compared with the phosgene group, MSCs decreased the levels of TNF-α, IL-1β, and IL-6 and increased the levels of IL-10, HGF and SP-C. Different MSCs displayed no significant differences in proliferation. However, BMSCs showed the strongest migration ability. HGF increased the expressions of p-GSK-3β and β-catenin in MSCs. All MSCs exerted protective effects on phosgene-induced lung injury by reducing inflammation, immune regulation and restoring cell function. Conclusions BMSCs showed the best protective effects followed by hCMSCs and LRMSCs. HGF enhances the role of MSCs by activating the p-GSK-3β/β-catenin signaling pathway.

Drowning is a crucial public safety problem and is the third leading cause of accidental fatality, claiming ~372,000 lives annually, worldwide. In near-drowning patients, acute lung injury (ALI) or acute respiratory distress syndrome (ARDS) is one of the most common complications. Approximately 1/3 of near-drowning patients fulfill the criteria for ALI or ARDS. In the present article, the current literature of near-drowning, pathophysiologic changes and the molecular mechanisms of seawater-drowning-induced ALI and ARDS was reviewed. Seawater is three times more hyperosmolar than plasma, and following inhalation of seawater the hyperosmotic seawater may cause serious injury in the lung and alveoli. The perturbing effects of seawater may be primarily categorized into insufficiency of pulmonary surfactant, blood-air barrier disruption, formation of pulmonary edema, inflammation, oxidative stress, autophagy, apoptosis and various other hypertonic stimulation. Potential treatments for seawater-induced ALI/ARDS were also presented, in addition to suggestions for further studies. A total of nine therapeutic strategies had been tested and all had focused on modulating the over-activated immunoreactions. In conclusion, seawater drowning is a complex injury process and the exact mechanisms and potential treatments require further exploration.
