Introduction: Hydatid disease of the bone is a parasitic infestation in the form of cystic echinococcosis by a tapeworm echinococcus granulosus. Its manifestation in the bone is relatively low and has the ability to mimic other skeletal pathologies. Case Report: We came across a 30-year-old male farmer with complaints of sero-purulent discharge from a sinus tract wound in the axilla for the past 3 years. He had been previously evaluated elsewhere with blood investigations, pus culture/sensitivity tests, MRI, and CT scans. Blood reports and culture/sensitivity tests were inconclusive and imaging tests were suggestive of a sinus tract originating from the right scapula. He was incorrectly diagnosed as a case of Tuberculosis of the Scapula and started on anti-tubercular medications for 9 months which was ineffective. To confirm the diagnosis, a biopsy was taken from the scapular lesion through a posterior approach. Histological tests revealed a cystic lesion composed of trilaminar membrane consisting of dead and degenerating scolices consistent with “Hydatid Cyst of Bone.” Conclusion: Hydatid disease of the bone has an inconsistent clinical picture which makes diagnosing this rare disease even more difficult. A high suspicion for hydatid infestation in bone pathologies could help in diagnosing the disease at the earliest.

Hydatid disease (HD), also known as echinococcal disease or echinococcosis, is a worldwide zoonosis with a wide geographic distribution. It can be found in almost all parts of the body and usually remains silent for a long period of time. Clinical history can be varied based on the location, size, host immune response, and complications.The most common imaging modalities used for diagnosis and further evaluations of HD are ultrasound, computed tomography (CT), and magnetic resonance imaging (MRI). Although conventional radiography may be the first used tool, rarely can lead to a definite judgment. Clinical indications and cyst location may alter the choice of imaging. MRI and CT would be useful when the involved area is inaccessible for ultrasound or surgical treatment is required. CT is particularly valuable for osseous organ involvements and the presence of calcifications in the cyst and also demonstrates the size, number, and local complications. MRI can differentiate HD from neoplasms in cases with an unusual appearance on imaging. Moreover, it is preferable in biliary or neural involvements. Besides, more detailed images of MRI and CT could help to resolve the diagnostic uncertainty.Imaging is the main stem for HD diagnosis. Brain, orbit, muscle, bone, and vascular structures are less commonly involved areas. Familiarity with typical clinical presentation, CT scan and MR imaging findings of HD in this sites facilitate the radiologic diagnosis and guiding appropriate treatment.

Önemli bir halk sağlığı sorunu olan kist hidatik, toplumda sık görülmekle birlikte çoğunlukla asemptomatik seyredebilmektedir. Yayılım gösterdiği organlar içinde rüptür olduğunda, hayatı ciddi bir şekilde tehdit etmekte ve ani solunum yetmezliği ile hastanın ölümüne neden olabilmektedir. Acil klinikte kist hidatik rüptürüne az rastlanmakta ve bu nedenle hastalar gözden kaçabilmektedir. Bu olguda, hasta perde asarken merdivenden kafatası üzerine düşme ile acil kliniğe başvurmuştur. Ön frontalde şişlik ve ödem şikâyeti olan hastanın takibinde anaflaktik şok bulguları gelişmiştir. Yapılan tetkik (tam kan, biyokimya, beyin ve toraks bilgisayarlı tomografi, akciğer grafisi) ve fizik muayeneler sonucunda hasta kist hidatik rüptürü tanısı almıştır. Bu olgu sunumu, acil kliniğe başvuran hastada geliş şikayetinden farklı bir semptom görülebileceğini ve travmanın nelere yol açabileceğini vurgulamayı amaçlar. Acil kliniğe başvuran hastanın şikayeti ne olursa olsun iyi bir fiziksel değerlendirme ve gözlemleme acil hemşiresinin görevlerindendir. Acil hemşiresi hastadaki semptomları yakından takip etmeli ve değişikleri hemen fark edebilmelidir.
