Body Positioning and Medical Therapy for Infantile Gastroesophageal Reflux Symptoms

Abstract: "Symptomatic gastroesophageal reflux disease" implies disease causation for distressing infant symptoms. In infants with symptoms attributed to GER, LLP produced a significant reduction in total GER, but did not result in a significant improvement in symptoms other than vomiting; however, automated analysis appeared to identify infants with GER-associated crying symptoms who responded to positioning therapy. This is an important new insight for future research.

“…It is well known that posture has an impact on the frequency of LOS relaxations, one of the physiological mechanisms to increase GOR episodes . In infants with symptoms attributed to GOR, prone and left lateral position produced a significant reduction in total GOR, compared to supine and right lateral position . Prone posture has been associated with sudden infant death and is not recommended .…”

“…If no response to conventional GERD treatment is observed, a trial of extensively hydrolyzed formula, amino acid–based formula, or breastfeeding after 2 weeks of diet elimination in mother are indicated as symptoms of GERD, and cow's milk protein allergy can be identical . Positioning therapy such as head elevation or left lateral positioning may be considered in children but not in sleeping infants because of risk of sudden infant death syndrome …”
