Objective: To determine frequency of electrolyte imbalance including sodium, chloride, potassium and magnesium levels present in patients with uncontrolled diabetes at a tertiary care hospital in Karachi. Methods: This was a descriptive cross sectional study conducted at Medicine Department, Darul Sehat Hospital, Karachi, Pakistan from March 2017 to October 2017. A total of one hundred and eighty one admitted and OPD patients with uncontrolled diabetes (HbA1c more than 7%) were included and their demographics, comorbidities, microvascular complications, drug history, fasting and random blood glucose and serum electrolyte levels were recorded. Results: In uncontrolled diabetes mellitus, decrease in serum sodium and chloride levels were observed to be statistically highly significant (p-value less than or equal to 0.05) while that of potassium and magnesium showed insignificant alterations. Sodium level was also observed to decline with increasing pattern of urine for microalbumin. Conclusion: Electrolyte imbalance is markedly present in patients with uncontrolled blood sugars therefore serum electrolytes should be routinely measured in patients with type 2 diabetes mellitus. Aims: To investigate the effects of Cucumis sativus and Prunus armeniaca juices on blood glucose, insulin and electrolytes of alloxan-induced Diabetic male Sprague Dawley rats. Study Design: An experimental study involving a total of eighty (80) male Sprague Dawley rats was used. Place and Duration of Study: Department of Medical Laboratory Science, Rivers State University, Nigeria, between January 2021 and February 2022. Methodology: 80 male Sprague Dawley rats were grouped into eight groups of ten rats each and housed in separate cages in an animal house. They were induced for diabetes with 150 mg/kg alloxan monohydrate intraperitoneally apart from group 1. They were then treated with various doses of apricot and cucumber juices. The juices were extracted by blending the pulp of each fruit using an electric blender and the juice sieved into a water bottle. Group 1 was the positive control, group 2 negative control, group 3 high dose apricot, group 4 low dose apricot, group 5 high dose cucumber, group 6 low dose cucumber, group 7 a combination of apricot and cucumber juices, group 8 metformin. The rats were treated for a period of 5 weeks and sacrificed. Blood glucose was estimated using the glucose oxidase method, electrolytes (Sodium, Potassium, Chloride) measured using serum electrolytes analyser, insulin was measured using Crystal Chem’s Rat insulin ELISA method. Phytochemical analysis was done using Cleg Anthrone method for carbohydrate, Boham and Kocipaiabyazari test for flavonoid and Harbone method for alkaloids. Statistical analysis was done using GraphPad prism and results expressed as mean ±SD (P < 0.05). Results: The results showed that high dose cucumber significantly reduced (P = 0.03) blood glucose level (5.4 mmol/L) compared to positive control (10.5 mmol/L); apricot and cucumber juices improved insulin level significantly (2.03 µIU/ml) (P < 0.01) compared to positive control (0.77 µIU/ml); a combination of apricot and cucumber juices improved potassium level (3.73 mmol/L) significantly (P = 0.03) compared to control (3.4 mmol/L). High Dose Cucumber also significantly reduced blood glucose level better than high dose apricot (P < 0.01). Conclusion: Cucumber juice therefore improved diabetic condition in diabetic male Sprague Dawley rats and a combination of apricot and cucumber juices boosted potassium level in diabetic rats.

Gongronema latifolium (GL) is reported to have hepato-protective and nephron-protective ability. Diabetes is associated with liver and kidney damage. This work was designed to evaluate the effect of ethanolic leaf extract of Gongronema latifolium on liver and kidney functions in Streptozotocin-induced diabetic rats. 40 Wistar rats of both sexes (150g-200g) were divided into five (n=8). Group 1- normal control (received normal saline placebo orally). Group 2 received 200mg/kg GL orally. Group 3 was the diabetic group administered 65mg/kg of streptozotocin (STZ) intraperitoneally for two consecutive days with an interval of one day in between. Group 4 (diabetic + GL group) had STZ (65mg/kg i.p.) + oral administration of 200mg/kg GL. Group 5 was the diabetic group treated with insulin subcutaneously. Blood samples were collected via cardiac puncture to assess Liver enzymes activity, Serum electrolytes biomarkers, Urea and Creatinine. The Result shows a significant (P<0.05) increase in the glucose level of the diabetic group compared to the control. Intervention with GL and insulin significantly reduced the glucose level towards normal. The liver enzymes activity was significantly (p<0.05) increased in the diabetic group but was significantly (p<0.01) decreased following treatment with GL and insulin. Sodium, potassium, chloride, urea and creatinine were significantly (p<0.01) increased in the diabetic group with a decrease in bicarbonate. GL and insulin significantly (p<0.01) reversed these changes in electrolytes and liver enzymes activity towards normal. Results presented shows that GL just like insulin possess hypoglycemic, nephro and hepato-protective potential. Thus, in diabetic condition, Gongronema latifolium leaf extract may exert ameliorative effect on these target organs.
