Blood-based Biomarkers Used to Predict Disease Activity in Crohnʼs Disease and Ulcerative Colitis

Abstract: The specific whole blood gene panels reliably distinguished CD and UC and determined the activity of disease, with high sensitivity and specificity in our cohorts of patients. This simple serological test has the potential to become a biomarker to determine the activity of disease.

“…Although CRP is used as a biomarker for disease activity in UC, it does not always reflect the mucosal inflammation. 31,32 In the present study, although most patients presented with normal CRP levels, the activation status of MAIT cells in the peripheral blood, such as the expression levels of CD69, correlated with clinical disease activity and endoscopic scores. Our findings suggest that MAIT cells may serve as a biomarker for mucosal inflammation in UC.…”

“…Recent therapeutic options for UC involve evaluation of mucosal inflammatory conditions in order to choose an effective treatment strategy. “…56 More recently, the Affymetrix GeneChip technology was utilized to generate genome-wide expression profiles to predict disease activity in patients with UC and CD. 57 In this study, whole blood gene panels determined the activity of disease with high sensitivity and specificity, while reliably distinguishing between UC and CD. 57 While these initial results are promising, the majority of studies evaluating the utility of whole blood gene expression analysis have been performed in small populations, and thus larger studies remain necessary for further evaluation of this modality.…”

“…However, besides being associated with significant procedural risks, colonoscopy is an invasive and expensive procedure. Therefore, non-invasive assessment is considered as the ideal mainstay of routine disease monitoring and many efforts have been made to identify laboratory biomarkers that can be used as an objective measure of disease activity and can help to diminish the need for colonoscopy 5 6…”
