<p class="abstract">Bleomycin is an antitumour antibiotic commonly used in the management of Hodgkin’s lymphoma and germ cell tumours. Flagellate pigmentation is a very specific and rare cutaneous adverse effect of bleomycin. It is characterised by linear, painless, pruritic, hyperpigmented lesions simulating marks of whiplashes over trunk and extremities. With the advent of targeted therapies resulting in the infrequent use of bleomycin, this unique side effect is also uncommonly encountered nowadays. Here, we report a case of bleomycin-induced flagellate pigmentation in a 35-year-old Indian female with ovarian teratoma. The characteristic patterned hyperpigmented patches developed following 2nd cycle of chemotherapy with bleomycin, etoposide and cisplatin.</p><p class="abstract"> </p>

Sclerotherapy with bleomycin can cause cosmetic complications, including flagellate dermatitis and hyperpigmentation, induced or exacerbated by microtrauma to the skin. We report a case of a 9-year-old pediatric patient with congenital vascular malformations in which a cohesive bandage (eg, 3M Coban) was utilized to prevent bleomycin-induced hyperpigmentation. Postoperatively and on follow-up, there were no signs of hyperpigmentation or dermatitis in our patient. This report highlights using skin protective measures during bleomycin sclerotherapy for improved postoperative outcomes. If a patient is undergoing bleomycin sclerotherapy, consider removing adhesive where possible and using cohesive bandage to secure lines, airway instruments, and monitoring equipment.

Objetivo: Describir nuestra experiencia en el tratamiento percutáneo de malformaciones linfáticas abdominales con doxiciclina en pacientes pediátricos. Materiales y métodos: Se analizaron retrospectivamente las historias clínicas de 6 pacientes (4 niños y 2 niñas), tratados de manera percutánea, con diagnóstico de malformación linfática abdominal, en el periodo comprendido entre los años 2012-2018. El rango de edad de presentación al momento del diagnóstico fue entre los 5 meses y 14 años. Las manifestaciones clínicas iniciales que motivaron la consulta fueron distensión, masa abdominal palpable, anemia y dolor intermitente. Todos los pacientes contaban con ecografía y tomografía computarizada o resonancia magnética abdominal previas al tratamiento percutáneo. Resultados: En todos los casos, el agente esclerosante utilizado fue doxiciclina. La malformación fue punzada bajo guía ecográfica y se inyectó un agente de contraste y posteriormente el agente esclerosante, bajo guía radioscópica. En tres pacientes se colocaron catéteres de drenaje en el seno de la malformación para su tratamiento percutáneo. Un paciente presentó un episodio febril después del procedimiento, que cedió con AINE. No se registraron complicaciones mayores. Los pacientes requirieron entre una y seis sesiones de esclerosis (teniendo en cuenta las sesiones a través de catéteres previamente colocados). Se utilizó un promedio de 400 mg de doxiciclina por paciente. En todos los casos hubo una buena respuesta al tratamiento, desapareciendo las manifestaciones clínicas durante el seguimiento y presentando una reducción significativa de las lesiones en las ecografías de control (>75 %, n=2) y resolución completa por ecografía en los casos restantes (n=4). El periodo de seguimiento posterior al procedimiento fue, en promedio, de 2 años.
