Blastic plasmacytoid dendritic cell neoplasm: challenges and future prospects

Abstract: Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is a rare CD4+ CD56+ myeloid malignancy that is challenging to diagnose and treat. BPDCN typically presents with nonspecific cutaneous lesions with or without extra-cutaneous manifestations before progressing to leukemia. Currently, there is no standard of care for the treatment of BPDCN and various approaches have been used including acute myeloid leukemia, acute lymphoblastic leukemia, and lymphoma-based regimens with or without stem cell transplantation. … Show more

“…We also find a positive association between T-cell exhaustion and T-cell clonal expansion, consistent with other cancers (21,51). A lack of TCR expression by malignant cells is one feature that distinguishes BPDCN from mature T-cell malignancies, such as cutaneous T-cell lymphoma (52,53). However, little research has been done on T-cell biology in BPDCN, and none at the single-cell level.…”

“…Interestingly, toxoplasma gondii infects and uses dendritic cells to spread the infection throughout the human body, and toxoplasmosis is furthermore considered a differential diagnosis to BPDCN [15,16]. Whether this connection is of any significance to this case report remains unclear due to the limited literature within the field, but a possible association between toxoplasmosis and hematological malignancies has earlier been suspected [17].…”

“…Nevertheless, instead of skin involvement, T-cell lymphoblastic lymphoma often presents with a mediastinal mass. As an extramedullary myeloid tumor, myeloid sarcoma consists of myeloblasts or premature myeloid cells [ 20 ]. Monoblastic lineage of both BPDCN and myeloid sarcoma express CD56, CD123, and CD4 markers; nevertheless, TdT is exclusively detected in 33% of the BPDCN patients.…”
