This third update, in conjunction with the International Society for Bipolar Disorders, reviews new evidence and is designed to be used in conjunction with the previous publications.The recommendations for the management of acute mania remain largely unchanged. Lithium, valproate, and several atypical antipsychotic agents continue to be first‐line treatments for acute mania. Monotherapy with asenapine, paliperidone extended release (ER), and divalproex ER, as well as adjunctive asenapine, have been added as first‐line options.For the management of bipolar depression, lithium, lamotrigine, and quetiapine monotherapy, as well as olanzapine plus selective serotonin reuptake inhibitor (SSRI), and lithium or divalproex plus SSRI/bupropion remain first‐line options. Lurasidone monotherapy and the combination of lurasidone or lamotrigine plus lithium or divalproex have been added as a second‐line options. Ziprasidone alone or as adjunctive therapy, and adjunctive levetiracetam have been added as not‐recommended options for the treatment of bipolar depression. Lithium, lamotrigine, valproate, olanzapine, quetiapine, aripiprazole, risperidone long‐acting injection, and adjunctive ziprasidone continue to be first‐line options for maintenance treatment of bipolar disorder. Asenapine alone or as adjunctive therapy have been added as third‐line options.

ResumoTrata-se de uma revisão sistemática da literatura na base de dados MEDLINE para avaliar os efeitos e o perfil de segurança do uso de lisdexanfetamina (LDX) em pacientes com transtorno depressivo (TD) e transtorno afetivo bipolar (TAB), utilizando apenas medical subject headings (MeSH). Foram utilizados os seguintes termos para a busca: "depressive disorder" [MeSH] OR "bipolar disorder" [MeSH] AND "lisdexamfetamine dimesylate" [MeSH]. Somente artigos em português e inglês foram selecionados, sem limitação de período. Como critério de inclusão para o nosso estudo, utilizamos apenas artigos que observaram perfil dos efeitos do uso de LDX em pacientes que apresentavam transtornos do humor, podendo apresentar transtorno de déficit de atenção e hiperatividade comórbido ou não. Foram selecionados oito artigos, mas apenas cinco foram incluídos na síntese narrativa. Encontramos poucos dados sobre o uso de LDX em transtornos do humor, mas os resultados demonstram um uso possível para potencializar o tratamento e reduzir prejuízos cognitivos em pacientes com TD e na melhora dos parâmetros da síndrome metabólica em pacientes com TAB.Palavras-chave: Transtorno bipolar, transtorno depressivo, dimesilato de lisdexanfetamina. AbstractThe present study is a systematic review of the literature published in the MEDLINE database to evaluate the effects and safety profile of lisdexamfetamine (LDX) use in patients with depressive disorder (DD) and bipolar disorder (BD), using only medical subject headings (MeSH). The following terms were used: "depressive disorder" [MeSH] . We selected articles published in Portuguese and English only, with no publication date restrictions. As an inclusion criterion, only articles that observed the profile of effects of LDX use in patients with mood disorders, with or without comorbid attention deficit and hyperactivity disorder, were included. Eight papers were selected, but only five were included in the narrative synthesis. We found few data on the use of LDX in mood disorders, but the results demonstrate a possible use to potentiate treatment and reduce cognitive impairment in patients with DD, as well as to improve metabolic syndrome parameters in patients with BD.Keywords: Bipolar disorder, depressive disorder, lisdexamfetamine dimesylate. Segundo a Organização Mundial de Saúde (OMS), a depressão é a segunda doença mais incapacitante do mundo, sendo a primeira quando consideramos o tempo vivido com a incapacitação no decorrer da vida 5 . Existe uma grande dificuldade na remissão completa dos seus sintomas, sendo o prejuízo cognitivo um dos principais sintomas residuais 6 . A hipótese do "cérebro egoísta" para alterações metabólicas no transtorno bipolar e na esquizofreniaThe "selfish brain" hypothesis for metabolic abnormalities in bipolar disorder and schizophrenia AbstractMetabolic abnormalities are frequent in patients with schizophrenia and bipolar disorder (BD), leading to a high prevalence of diabetes and metabolic syndrome in this population. Moreover, mortality rates among patients are higher than in the general population, especially due to cardiovascular diseases. Several neurobiological systems involved in energy metabolism have been shown to be altered in both illnesses; however, the cause of metabolic abnormalities and how they relate to schizophrenia and BD pathophysiology are still largely unknown. The "selfish brain" theory is a recent paradigm postulating that, in order to maintain its own energy supply stable, the brain modulates energy metabolism in the periphery by regulation of both allocation and intake of nutrients. We hypothesize that the metabolic alterations observed in these disorders are a result of an inefficient regulation of the brain energy supply and its compensatory mechanisms. The selfish brain theory can also expand our understanding of stress adaptation and neuroprogression in schizophrenia and BD, and, overall, can have important clinical implications for both illnesses. ResumoAlterações metabólicas são frequentes em pacientes com esquizofrenia e transtorno bipolar (TB), levando a uma alta prevalên-cia de diabetes e síndrome metabólica nessa população. Além disso, as taxas de mortalidade entre pacientes são mais altas do que na população geral, especialmente em decorrência de doenças cardiovasculares. Vários sistemas neurobiológicos envolvidos no metabolismo energético têm demonstrado alterações nas duas doenças; no entanto, a causa das alterações metabó-licas e a forma como elas se relacionam com a fisiopatologia da esquizofrenia e do TB ainda são arenas em grande parte desconhecidas. A teoria do "cérebro egoísta" é um paradigma recente que postula que, para manter estável seu próprio fornecimento de energia, o cérebro modula o metabolismo da energia na periferia regulando tanto a alocação quanto a ingestão de nutrientes. Apresentamos neste artigo a hipótese de que as alterações metabólicas observadas nesses transtornos são resultado de uma regulação ineficiente do fornecimento de energia do cérebro e seus mecanismos compensatórios. A teoria do cérebro egoísta também pode expandir nosso entendimento sobre a adaptação ao estresse e a neuroprogressão na esquizofrenia e no TB, e, acima de tudo, pode ter implicações clínicas importantes para as duas doenças.
