Bronchial asthma (BA) is a heterogeneous disease characterized by chronic inflammation of the airways. Severe asthma occurs in all age groups and represents a clinically and etiologically heterogeneous group, accounting for about 5-10% of patients with asthma. Achieving control over the disease is the main and important task of treating bronchial asthma in all its forms. Despite existing clinical guidelines for the treatment of asthma, studies show that approximately 50% of patients remain poorly controlled. This problem is also relevant in adolescence, especially in severe forms of the disease. Adolescent children with uncontrolled asthma have a poor quality of life, many absences from school due to frequent exacerbations or asthma symptoms, do not leave the house, lead a passive lifestyle, may have depression and other psychological and social disorders. Modern methods of asthma therapy, based on the molecular mechanisms and pathogenesis of the disease, allow solving the issues of providing effective medical care to patients with uncontrolled moderate and severe asthma. They allow you to achieve control over the symptoms of the disease, minimize the risks of future exacerbations. This article presents a clinical example of a teenager with severe uncontrolled asthma, with concomitant allergic persistent rhinitis and hay fever. Anti-IgE biological therapy with omalizumab was prescribed. In a short period of therapy, control of asthma symptoms is achieved. The frequency of asthma exacerbation was reduced, the use of systemic corticosteroids (SCS) until their complete cessation, the use of short-acting inhaled β2agonists (SABA) was reduced to a minimum, and the dosage of inhaled hormones (IGCS) was reduced. As a result, the quality of life of a teenager is improved, the levels of his physical and daily activity and socialization are increased.

Biological therapy of bronchial asthma (BA) is a modern method of treating severe forms of the disease, that are uncontrolled by traditional pharmacotherapeutic approaches. Currently, 5 monoclonal antibody (AT) preparations are registered in the world for the treatment of severe bronchial asthma (SBA) of the T2 endotype (T2-SBA) – antibodies, binding to immunoglobulin (Ig) E (anti-IgE – omalizumab), interleukin antagonists (IL)-5 (anti-IL-5 – mepolizumab, resizumab) and its receptor (anti-IL-5Rα – benralizumab), as well as antibodies, that selectively bind to the IL-4 and -13 receptor (anti-IL-4 /13Rα – dupilumab). The article presents data on the effectiveness of these drugs in relation to the key characteristics of SBA, formulates clinical and laboratory criteria, the study of which in real practice can potentially predict the likelihood of a clinical response to a particular type of biological therapy. An algorithm is proposed for choosing a targeted therapy strategy for patients with SBA, clinically associated with allergies, for patients with severe non-allergic eosinophilic BA and for patients with eosinophilic BA of a combined phenotype.

The emergence of new means of administering genetically-engineered biological drugs, such as an autoinjector (pen injector device), can positively affect the organizational aspects of treating patients with severe eosinophilic asthma (SA) who need biological therapy.The aim. To determine the place of a new delivery device for benralizumab (autoinjector, pen injector device) in the clinical practice of treating eosinophilic SA.Results. The expert council considered the results of the latest clinical studies and real practice data on the use of genetically-engineered biological drugs in the form of an autoinjector. The experts discussed the safety and efficacy of this delivery device and recommended considering the possibility of switching eosinophilic SA patients to self-administration of genetically-engineered biological drugs (autoinjector form) at home. This treatment tactic is especially relevant in the current epidemiological situation since it will reduce the risks of infection compared to planned medical care in inpatient or outpatient treatment settings and reduce the burden for healthcare workers.Conclusion. The practice of switching eosinophilic SA patients to self-administration of biologics in the form of an autoinjector (pen injector device) can improve the adherence to biological therapy and quality of life of this group of patients.
