Association of ADIPOQ Gene Polymorphism with Circulating Adiponectin Level in Iraqi Women with Gestational Diabetes Mellitus

Background Several adipokines are involved in the pathophysiology of gestational diabetes mellitus (GDM), we explored potential associations of the variant + 45T/G (rs2241766) in ADIPOQ gene with maternal circulating adiponectin, obesity and GDM risk in apparently healthy and GDM Iraqi pregnant women. Methods Participants including 50 normal pregnant women and 52 pregnant women with GDM were enrolled from December 2022 to May 2023 at Baghdad Teaching Hospital, Medical city. Serum adiponectin levels were measured by immunoassay. ADIPOQ + 45T/G (rs2241766) variant was determined through Taqman Real Time quantitative polymerase chain reaction RT-PCR technique. Results ADIPOQ + 45T/G polymorphism was significantly associated with an increased risk of GDM. The G allele frequency of ADIPOQ + 45T/G was significantly higher among GDM compared with non-GDM controls (odds ratio [OR]: 3.5; 95% confidence interval [CI]: 1.8931 to 6.6466; P = 0.0004). Serum adiponectin concentration was also significantly decreased among the GDM who had a BMI of less than 30 kg/m2. Conclusion This study revealed that ADIPOQ + 45T/G rs2241766 polymorphism was implicated in GDM onset. In our study, serum adiponectin levels were significantly lower in Iraqi pregnant women with GDM who had pregnancy BMI of < 25 Kg/m2. However, to elucidate the mechanism of adiponectin in predicting gestational diabetes mellitus, further studies are required.

Background The solute carrier family 30 A8 zinc transporter (SLC30A8) plays a vital role in insulin synthesis and secretion. This study aimed to investigate the association of rs13266634 C/T polymorphism in the Solute carrier family 30 member 8 (SLC30A8) gene with GDM risk among Iraqi pregnant women in addition to examine maternal serum zinc transporter 8 (ZnT8) and antibodies to ZnT8 (anti-ZnT8 Abs) levels during pregnancy both in women with or without GDM and their associations with SLC30A8 rs13266634 C/T SNP. Methods A case-control study was carried out from November 2022- May 2023 in Baghdad Teaching Hospital/ Pregnancy Care unit/ Baghdad- Iraq. The study sample included 17–43-year-old women; 52 pregnant women with GDM and 50 non-GDM control pregnant women. GDM was diagnosed by the obstetrician based on HbA1c test and ultrasound. GDM risk was tested in a multivariable model, adjusted for age, gravidity and different pregnancy complications. Blood samples were collected from all participants and were genotyped for the rs13266634 SNP by Taqman Real Time quantitative polymerase chain reaction RT-PCR technique. Results SLC30A8 rs13266634 C/T SNP was associated with an increased risk of GDM. The T allele frequency of SLC30A8 rs13266634 C/T SNP was significantly different between GDM and controls (odds ratio [OR]: 2.4; 95% confidence interval [CI]: 1.3535 – 4.2959; P = 0.002). TT genotype versus CC genotype was significantly associated with higher GDM risk (OR = 48; 95% CI: (2.5235 to 91.7594); P = 0.01), and subjects with CT+TT genotype were more likely to have GDM compared to those with CC genotypes, (OR =3.2; 95% CI: 1.2475 – 8.2995; P = 0.009). Serum ZnT8Ab concentration was significantly increased among the GDM and non-GDM who had CT genotype (OR =6.500; 95% CI: 1.787- 23.644; P = 0.003). Conclusions Our study shows that Iraqi pregnant women who are T allele carriers have a higher risk of GDM unlike C allele carriers. ZnT8Ab is protective from GDM while ZnT8 is an independent risk factor which was significantly higher in the GDM subjects.

Background: The human zinc transporter 8 (ZNT8), also known as the solute carrier family 30A, member 8 (SLC30A8), is a β-cell specific integral membrane protein, mainly restricted to the membrane of insulin secretory granules. ZnT8 plays a crucial role in both insulin synthesis and secretion as well as the viability of β-cells. The recently identified autoantibodies targeting the β-cell surface epitopes of ZnT8 have been proposed as a new risk factor for gestational diabetes mellitus (GDM). Due to the lack of prior research studies on the role of β-cell surface autoantibodies against ZnT8 in the immunologic etiology of GDM in Iraqi population, this study has been designed to explore the association between the rs13266634 C>T polymorphism in the SLC30A8 gene and the serum levels of ZnT8 as well as Zinc Transporter-8 Autoantibody (ZnT8A) in women diagnosed with GDM. Methods: A total of 50 participants with GDM and 50 controls from the Baghdad Teaching Hospital/Pregnancy Care unit at the Medical City, Baghdad, Iraq, were recruited in this case–control study. Serum levels of ZnT8 and ZnT8A were assayed with enzyme-linked immunosorbent assay, and Taqman real time polymerase chain reaction (RT-PCR) was used for rs13266634 single nucleotide polymorphism (SNP) genotyping. Results: ZnT8A levels were significantly lower in women with GDM than non-GDM women (P ≤ 0.01). A ZnT8A level lower than 20.0 pg/mL was significantly associated with a threefold higher risk for GDM with P ≤ 0.001. ZnT8 levels were significantly higher in the GDM subjects (P ≤ 0.05). The T allele at the rs13266634 C>T SNP significantly conferred a higher risk of GDM (P = 0.002). In view of that, the TT genotype, compared to the CC genotype, showed a significant association with increased GDM risk (P ≤ 0.01). Serum ZnT8A concentrations were significantly low among GDM women who had CT genotype (P = 0.003). Conclusion: The T variant at the SLC30A8 rs13266634 C>T polymorphism appears to play a key role in determining the β-cell autoimmunity by inducing an autoantibody response due to its hyperactivity of zinc transportation.
