Introduction:In clinical practice, monitoring body composition is a critical component of nutritional assessment and weight management in boys with Duchene muscular dystrophy (DMD). We aimed to evaluate the accuracy of a simple bedside measurement tool for body composition, namely bioelectrical impedance analysis (BIA), in boys with DMD. Methods:Measures of fat free mass (FFM) were determined using a BIA machine and compared against estimations obtained from a reference body composition model. Additionally, the use of raw impedance values were analysed using three existing predictive equations 1-3 for the estimation of FFM. Accuracy of BIA was assessed by comparison against the reference model by calculation of biases and limits of agreement.Results: Body composition was measured in ten boys with DMD, mean age 9.01 ± 2.34 years. The BIA machine values of FFM were on average 2.3 ± 14.1 kg higher than reference values. Limits of agreement (based on 95 % CI of the mean) were -7.4 to 2.9 kg. There was a significant correlation between the mean FFM and difference in FFM between the BIA machine and the reference model (r = -0.86, p= 0.02) suggesting that the bias was not consistent across the range of measurements. The most accurate predictive equation for the estimation of FFM using raw impedance values was Pietrobelli's 3 ; mean difference -0.7 kg, 95 % limits of agreement (-3.5 to 2.0 kg). Conclusion:In a clinical setting, where a rapid assessment of body composition is advantageous, the use of raw impedance values, combined with the Pietrobelli 3 equation, is recommended for the accurate estimation of FFM, in boys with DMD.

RESUMOIntrodução: Considerada um problema de saúde pública, a obesidade tem sido referida como fator de risco para o desenvolvimento da asma. Assim, o excesso de peso apresenta influencia sobre a função da musculatura lisa e induz a inflamação sistêmica das vias aéreas por intermédio das adipocitocinas. Embora a adiponectina seja considerada uma citocina de caráter anti-inflamatório, uma vez que, suas concentrações decrescem na obesidade. A literatura tem demonstrado mínimas evidências do comportamento dessa adipocitocina perante os fatores envolvidos na relação de obesidade e asma. Desse modo, o presente estudo visou analisar o comportamento das respostas de adiponectinemia e parâ-metros fisiológicos em adolescentes obesos asmáticos e não-asmáticos. Métodos: Participaram 15 adolescentes obesos (seis asmáticos e nove não-asmáticos), submetidos a avaliação antropométrica, cardiorrespiratória, espirométrica e laboratorial. O diagnóstico de asma foi através de histórico clínico e questionário ISAAC, e a obesidade pelo IMC acima do percentil 95 th . Utilizou-se o teste de broncoprovocação com exercício físico para avaliação do broncoespasmo induzido pelo exercício (BIE). Resultados: Verificou-se diferença do % Queda VEF 1 (p=0,034) entre os obesos asmáticos e não-asmáticos. Para o obesos asmáticos, o coeficiente de determinação (R 2 ) demonstrou efeito da adiponectina com a PAS (r=-0,512;R 2 =0,105); HDL (r=+0,659;R 2 =0,292), glicemia (r=-0,599;R 2 =0,199), glicemia120 (r=-0,686;R 2 =0,338), insulina120 (r=-0,614;R 2 =0,221), VO 2máx Absoluto (r=+0,518;R 2 =0,086), VO 2máx Relativo MLG (r=+0,585;R 2 =0,178) e %VEF 1 (r=+0,580;R 2 =0,171). Nos obesos não-asmáticos houve efeitos da adiponectina com a insulina (r=-0,731;R 2 =0,525), HOMA-IR (r=-0,684;R 2 =0,469), QUICKI (r=+0,683;R 2 =0,535) e VO 2máx Relativo MLG (r=+0,654;R 2 =0,346). Conclusão: Verificaram-se prováveis efeitos das respostas de adiponectinemia com os parâmetros fisiológicos associados ao excesso de peso e o baixo nível de aptidão cardiorrespiratória em obesos asmáticos e não-asmáticos. Além disso, para os obesos asmáticos a adiponectinemia demonstrou um possível efeito de relação com o %VEF 1 .Palavras-chave: Adiponectina. Asma. Obesidade.

Objective: To determine the impact of commonly used body fat percentage (%BF) references when evaluating the sensitivity, specificity and misclassifications of body mass index (BMI) for obesity screening in adolescence. Subjects/Methods: A community-based sample of 3334 adolescents aged 15.270.6 years was studied. Weight, height and %BF were measured. Sensitivity, specificity, predictive values and type and extent of misclassifications were calculated. Truepositive subjects for overweight and obesity were defined by recently published paediatric %BF cutoffs, 85th and 95th percentiles from the present study population, and an older %BF reference. Results: Regardless of %BF reference used, specificities were high (40.98 for obesity and 40.90 for overweight) in both sexes, with values for overweight consistently higher for girls (Po0.05). Sensitivities were much lower, with boys ranging from 0.52 to 0.74 for overweight, and 0.24 to 0.43 for obesity. Corresponding sensitivities for girls were 0.42-0.66 and 0.22-0.46. Large discrepancies were seen in total number of misclassifications, with published references resulting in twice as many misclassified for overweight (620, 514 vs 362) when compared to the cutoffs from the study population. For obesity, the difference was even greater (367 vs 133). Conclusions: No matter which %BF reference used, specificity of BMI was high, and sensitivity low for both sexes. Population percentiles resulted in somewhat lower specificity but much higher sensitivity, and many more correctly classified compared to when cutoffs from the literature were used. The choice of %BF reference in evaluations of BMI-based classification systems has a large impact on the outcome. Sensitivity analyses are therefore warranted.
