Raised circulating concentrations of asymmetric dimethylarginine (ADMA), an endogenous inhibitor of nitric oxide synthase (NOS), have been reported in several rheumatic diseases (RDs). However, the strength of this relationship is unclear. Therefore, the aim of this systematic review and meta-analysis was to evaluate the magnitude and the robustness of the association between ADMA concentrations and RDs. We calculated standardized mean differences (SMD, with 95% confidence intervals, CI). Study heterogeneity was evaluated by meta-regressions and sensitivity analyses according to type of RDs, conventional cardiovascular risk factors, inflammatory markers, and type of ADMA assessment methodology. Thirty-seven studies with a total of 2,982 subjects (1,860 RDs patients and 1,122 healthy controls) were included in our meta-analysis. Pooled results showed that ADMA concentrations were significantly higher in patients with RDs than in healthy controls (SMD = 1.27 µmol/L, 95% CI 0.94–1.60 µmol/L; p < 0.001). However, the between-studies heterogeneity was high. Differences in ADMA concentrations between controls and RDs patients were not significantly associated with inflammatory markers, increasing age, lipid concentrations, body mass index, blood pressure, or methodology used to assess ADMA. Furthermore, subgroup analysis showed no difference across RDs. This meta-analysis showed that, in the context of significant between-study heterogeneity, circulating concentrations of ADMA are positively related to RDs.

Objectives: Systemic Lupus Erythematosus (SLE) is a complex, multisystem autoimmune inflammatory disease. The inflammatory response in SLE exacerbates oxidative stress which tends to promote lipid peroxidation, protein oxidation and damage to DNA. Documentation of manifestation of DNA damage in SLE patients is scarce and related studies have not come to attention from this part of the world, therefore the present study showcases a case report on SLE. Methods: Assessment of DNA damage in peripheral blood leukocytes and oxidative stress biomarkers in blood sera of a 12y-old male presenting with SLE and nephropathy (on dialysis therapy) was made. Results: Basal DNA damage in PBL scored as percent DNA in tail was higher (51.48%) compared to levels (40.38%) in an age-and sex-matched healthy control. Quantitative measures of DNA damage revealed DF of 96 vs. 94 and DI of 317 vs. 208 in the case and the control, respectively. Serum lipid peroxidation as estimated from MDA level was 3x higher (1.759µmol/l) compared to its level in a healthy control (0.571µmol/l). OSI was slightly higher in the patient (0.134 arbitrary units) compared to that in the control (0.132 arbitrary units). The atherogenic indices were higher in the present case compared to ratios in the control. Conclusion:The results from the case report on increased oxidative stress, dyslipidemia, genetic damage and atherogenic indices support observations from earlier studies and imply onset of other complications in patients with SLE. The assessment of various blood-based biomarkers (as carried out in the present case) in SLE patients may assist in disease diagnosis, prognosis and optimal disease management.

ObjectiveSystemic lupus erythematosus (SLE) is an autoimmune disease with inflammation as a critical feature. Recently, high‐density lipoprotein cholesterol (HDLc) have been evidenced to have anti‐inflammatory effects, suggesting a potential link between HDL and SLE that needs to be thoroughly studied. The aim was to explore the association between SLE and HDLc through a systematic review with meta‐analysis.MethodsA systematic review with meta‐analysis was conducted to assess mean differences in HDL levels between patients with SLE and healthy controls. Both qualitative and quantitative syntheses were performed, including an assessment of heterogeneity using I2, a publication bias evaluation, a methodologic quality assessment, and a forest plot under a random effects model. Subgroup analyses were conducted based on disease activity and the report of corticosteroid dosage.ResultsA total of 53 studies were included in the qualitative synthesis, and 35 studies were included in the quantitative synthesis, comprising 3,002 patients with SLE and 2,123 healthy controls. Mean HDL levels were found to be lower in patients with SLE as follows: in the meta‐analysis including all articles −6.55 (95% confidence interval [CI] −8.77 to −4.33); in patients with mild disease activity −5.46 (95% CI −8.26 to −2.65); in patients with moderate or severe disease activity −9.42 (95% CI −15.49 to −3.34); in patients using corticosteroids −5.32 (95% CI −10.35 to −0.29); and in studies with excellent methodologic quality −8.71 (95% CI −12.38 to −5.03).ConclusionHDL levels appear to be quantitatively altered in patients with SLE, suggesting a potential contribution to immune dysregulation, highlighting the importance of HDL in autoimmune diseases.
