The advantages of sequential bilateral cochlear implantation were assessed in 29 children with a severe to profound hearing loss. The effect of age at second implantation and the effect of duration of bilateral implant use on the outcomes in speech perception and directional hearing were investigated. The children received their second cochlear implant at an age ranging from 2.8 to 8.5 years. Measurements were carried out preoperatively and postoperatively after 6, 12 and 24 months of bilateral implant use. A matched control group of 9 children with a unilateral implant were also measured over time and were compared with the study group after 12 and 24 months. Speech reception in both quiet and in noise and lateralization were measured. After 24 months, a minimum audible angle task was carried out. Bilateral advantages with regard to speech reception in quiet and in noise were already present after 6 months of bilateral implant use and improved thereafter. After 24 months, speech reception in noise had significantly improved with bilateral implants compared to that of children with a unilateral implant. The percentage of children that could accurately lateralize increased from 57% after 6 months to 83% after 24 months. With regard to the minimum audible angle task, loudspeakers were placed on average at ±42°. Age at second implantation did not have an influence on all outcomes. From the results it can be concluded that the advantages of bilateral hearing occur after sequential bilateral implantation and that age at second implantation does not influence the amount of bilateral advantage. Furthermore, it can be concluded that longer periods of bilateral implant use lead to greater bilateral advantages.

RESUMO -O presente estudo exploratório pretende investigar as capacidades linguísticas expressivas de crianças surdas com implante coclear por meio de amostras de linguagem espontânea. Para tal, será calculada a Extensão Média do Enunciado em palavras, e analisado o uso produtivo de morfemas gramaticais. Analisou-se o discurso espontâneo de três crianças portuguesas surdas com implante coclear e de três crianças normo-ouvintes, emparelhadas caso a caso, tendo em conta a idade cronológica, o género e a escolaridade parental. As crianças surdas com implante coclear produziram em média menos 2 palavras por enunciado que as crianças normo-ouvintes. Na morfologia, as crianças com implante coclear da amostra revelaram ainda não ser capazes de produzir plurais irregulares e de usar produtivamente os morfemas flexionais nos modos conjuntivo e imperativo. Em geral, o progresso gramatical parece ser mais lento para crianças surdas com implante coclear quando comparado ao dos seus pares ouvintes. Porém, uma implantação precoce, um maior período de tempo com o implante e uma implantação bilateral parecem trazer benefícios para a linguagem expressiva nesta população. For this purpose, Mean Length of Utterance in words will be measured and the productive use of single grammatical morphemes will be analysed. We analysed the spontaneous speech of three deaf Portuguese children with cochlear implants and three normal-hearing children paired case by case, taking into account the chronological age, gender and parental education. Cochlear-implanted children produced on average less than 2 words per utterance than normal hearing children. In morphology, cochlear implanted children from the sample were not capable to produce irregular plural formatives and to make productive use of inflectional morphemes on main verbs for the subjunctive and imperative modes. In general, the grammatical progress seems to be slower for cochlear-implanted children when compared to their hearing peers. Nevertheless, an earlier implantation, longer implant use and a bilateral implantation seem to bring benefits for the child's spoken language.

Objective: To identify and describe predictors of pediatric cochlear implantation outcomes in a South African population. Methods:A retrospective study of 301 pediatric CI recipients from five cochlear implant programs was conducted and cross-sectional outcome data were added at the time of data collection. Twenty potential prognostic factors were identified from the retrospective dataset, including biographical, cochlear implant (CI), family and risk factors. Multiple regression analyses was performed to identify predictor variables that influence outcomes in terms of auditory performance (CAP scores), speech production (SIR scores), communication mode and educational placement.Results: Although implanted children within this sample did not have equal opportunity to access a second implant, bilateral implantation was strongly predictive of better auditory performance and speech production scores, an oral mode of communication and mainstream education. NICU admittance/ prematurity were associated with poorer auditory performance and speech production scores, together with a higher probability for non-oral communication and non-mainstream education. The presence of one or more additional developmental 2 condition was predictive of poorer outcomes in terms of speech production and educational placement, while a delay between diagnosis and implantation of more than one year was also related to non-mainstream education. Ethnicities other than Caucasian were predictive of poorer auditory performance scores and a lower probability for mainstream education.Conclusion: An extensive range of prognostic indicators were identified for pediatric CI outcomes in South Africa. These predictive factors of better and poorer outcomes should guide pediatric CI services to promote optimal outcomes and assist professionals in providing evidence-based informational counselling.
