Bidirectional-Barbed Sutured Knotless Running Anastomosis v Classic Van Velthoven Suturing in a Model System

Abstract: It appears from our preliminary work that a bidirectional-barbed suture might improve the vesicourethral anastomosis during a robotic radical prostatectomy. Further investigations should be done to measure the disruptive force necessary to distract these sutures, whether the applied forces of the barbs are adequate for maintaining a watertight seal, and the reproducibility of our results by other surgeons. All of these investigations are in progress in our laboratory.

“…Anastomosis can be started with the new suture. In an experimental study by Moran et al [21], cutting of this suture after every 4 throws did not loosen the anastomosis. -After successful completion of anastomosis, the tension is taken up by the anchors and anastomosed tissues remain free from tension.…”

“…Only immediate perioperative data have been assessed in these studies and as such 2 and 5 month follow-up for continence or bladder neck contractures are not reported (21)(22)(23)(24). In the comparative studies, no data is provided for urinary incontinence between patients undergoing the UVA with barbed versus standard monofilament suture.…”

“…However, theories on the potential improvements of barbed suture versus monofilament suture were discussed. Moran et al stated that a bidirectional barbed suture allows for minimal tension during the UVA, resulting in less inflammation, scar formation, and tissue ischemia (24).…”

“…Greenberg et al described that the bidirectional barbs of the Quill TM device introduce a revolutionary technology in which wound tension is evenly distributed across the entire length of the suture line, eliminating the need for interrupted sutures or knot tying [21] . The Quill TM has now been applied in vesicourethral anastomosis in robotic prostatectomy [22] , closure of the collecting system and renal parenchyma during laparoscopic porcine partial nephrectomy [17] , and laparoscopic myomectomy and total laparoscopic hysterectomy [23] .…”
