SUMMARYThe efficacy of electrical defibrillation is considered to be related to the autonomic status. In search of a possible adjunct to enhance the therapeutic performance of an implantable cardioverter-defibrillator, we investigated whether parasympathetic manipulation by cervical vagal nerve stimulation (VNS) increases defibrillation efficacy. The effects of VNS on transcardiac defibrillation threshold (DFT) were assessed in 55 anesthetized dogs. In neurally intact dogs, right and left unilateral VNS at 10 mA for 7 seconds significantly decreased the DFT after 10 seconds of ventricular fibrillation (control: 3.1±0.9 J, right: 2.1±0.9 J [∆-35±12%, P<0.0001], left: 2.2±0.8 J [∆-31±11%, P<0.0005]), while bilateral VNS did not (2.8±1.0 J). In dogs with decentralized vagus nerves, both unilateral and bilateral VNS decreased the DFT. The extent of the VNS-induced decrease in DFT was dependent on the current and the duration of stimulation. We conclude that unilateral VNS decreases the DFT, while bilateral VNS paradoxically has no effect on the DFT unless the vagi are decentralized. (Jpn Heart J 2003; 44: 91-100)

AtualizaçãoO tratamento profilático da morte súbita sofreu, nos úl-timos 15 anos, profunda reformulação. A utilização de fár-macos antiarrítmicos, maior alternativa disponível até a primeira metade da década de 1980, mostrava-se pouco eficaz, ineficaz ou mesmo deletéria, servindo de cenário para o desenvolvimento dos cardioversores desfibriladores implantáveis. Em função de sua eficácia e segurança, essas próte-ses tornaram-se a primeira opção de tratamento na profilaxia secundária de morte súbita e, em algumas situações, na profilaxia primária.Entretanto, mesmo em portadores de cardioversor desfibrilador implantável, o uso de drogas antiarrítmicas pode não ser dispensável. Algumas situações tornam necessária a terapia farmacológica associada, tais como: 1) prevenção de taquicardia sinusal que motive terapias inapropriadas do aparelho, frente a circunstâncias em que a freqüência cardíaca atinja níveis superiores ao limite inferior estabelecido de corte para o qual o sistema esteja programado; 2) prevenção da ocorrência de taquicardias supraventriculares, como a fibrilação atrial que, além das implicações clínicas, pode também ser responsável por terapias inapropriadas do aparelho; 3) redução da resposta ventricular em portadores de fibrilação atrial permanente, quando os aumentos transitórios da freqüência cardíaca ocasionam terapias inapropriadas; 4) redução do número de recorrências de taquicardia ventricular, tendo como meta evitar terapias freqüentes pelo sistema que, por sua vez, podem implicar em má qualidade de vida e desgaste prematuro da bateria do aparelho.As drogas antiarrítmicas podem alterar os efeitos de um choque sobre o miocárdio ventricular, no sentido de elevar ou reduzir os limiares de cardioversão ou desfibrilação, o que tem suscitado um grande debate sobre a segurança de sua utilização em portadores de cardioversor desfibrilador implantável. Drogas antiarrítmicas e limiares de desfibrilaçãoQuando se analisam as diversas pesquisas, já realizadas, sobre interação entre fármacos antiarrítmicos e limiares de desfibrilação, constatam-se freqüentemente resultados contraditórios. A razão para tais discrepâncias está, possivelmente, relacionada à heterogeneidade metodológica na realização dos estudos 1 . Os tipos de modelos experimentais e de cardiopatias, os formatos de ondas de choque empregados, os protocolos de avaliação de limiares, além das doses e vias de administração das drogas antiarrítmicas, são notadamente distintos, de série para série. A interferência dos agentes anestésicos sobre os efeitos da desfibrilação, assim como a própria dificuldade prática de se diferenciar uma fibrilação ventricular de uma taquicardia ventricular polimórfica, constituem provavelmente fatores adicionais da inconsistência nas conclusões dos experimentos. The impact of acute volume overload hemodynamics on the DFT with concurrent moderate left ventricular systolic dysfunction is unknown. Ten mongrel dogs (17.2 kg), using a crossover study design, each had baseline (study 1) hemodynamic, echocardiographic, and DFT measurements. These measurements were repeated after left ventricular dysfunction was induced using a norepinephrine (5 micrograms/kg per min) infusion (study 2). Hemodynamic and DFT parameters were obtained simulating acute heart failure by volume overload with an 0.9% normal saline infusion to an associated mean pulmonary capillary wedge pressure of > 19 mmHg (study 3). Numerous significant echocardiographic and hemodynamic parameters were noted when the animals from studies 1 and 2, 2 and 3, and 1 and 3 were compared. A significant difference in the DFT was observed only when study animals 1 and 3 were compared (P < 0.02). None of the dogs were hypoxemic at the time of the acute heart failure DFT. The mechanism of this finding remains unknown. This data would suggest that acute volume overload with associated left ventricular dysfunction does adversely affect the DFT in a canine model.
