Belatacept during pregnancy in renal transplant recipients: Two case reports

Abstract: Impaired fertility is common among patients with chronic organ failure, including end-stage renal disease (ESRD). Women of childbearing age undergoing transplantation may experience rapid return of fertility. Pregnancy posttransplant presents numerous risks for the patient, fetus, and allograft. Maternal risks include hypertension and preeclampsia. Allograft risks include acute rejection and failure of the organ, and fetal risks include miscarriage, birth defects from immunosuppressants, premature delivery, an… Show more

“…The literature is rich with reports on extreme situations, including kidney transplantation during pregnancy; while single cases cannot lead to potentially risky changes in daily practices, such cases do warn against fully negative attitudes and suggest an individually based decision [ 145 , 146 , 147 , 148 , 149 , 150 , 151 ].…”

“…This case supports the 2 previously reported pregnancies in kidney transplant recipients on belatacept. (4) What stands out is that our patient, who was treated with belatacept and minimal standard immunosuppression, did not experience any of the frequent and serious complications described by previous authors (1,2) and in the Transplant Pregnancy Registry. (3) We did not lower or discontinue her already minimal standard immunosuppression, leaving her on belatacept alone, because she had developed cAMR in 2 liver grafts that was controlled by adding belatacept.…”

“…Information regarding the safety of immunosuppressant drugs in pregnancy accumulates slowly and is hampered by the absence of comprehensive post marketing ascertainment of pregnancy outcomes in women treated with immunosuppressant medications. For example, belatacept is available but its association and safety with pregnancy is known only from case reports [24]. Infertility due to chronic illness and patients choosing to use effective birth control options could be other contributing factors to lower rates of pregnancy in kidney transplant recipients as compared to general population.…”
