Basal cell carcinoma with monster cells

Abstract: A case of basal cell carcinoma with "monster cells" is reported. Clinically, the lesion presented as a red nodule on the forearm of a 58-year-old male. The histologic picture was striking, with large, "monstrous" nuclei scattered throughout a well defined nodule. Basal cell carcinoma with monster cells appears to represent a histologic variant of nodular basal cell carcinoma. The prognostic significance of the monster cells remains to be established.

“…All cases evaluated by static image analysis cytometry have proven to be aneuploid 49 but paradoxically, mitoses, although present with roughly the same frequency as seen in typical nodular BCCs, are not often atypical. 48,49 As these pleomorphic monster cells impart no prognostic significance from the standpoint of enhanced biological aggressiveness, we speculate that this represents a form of senescent atypia.…”

“…These are termed pleomorphic BCC or 'basal cell epithelioma with monster cells'. 48 Typically, these mis-shapen giant nuclei are either scattered individually through tumor lobules or show clustering suggesting that they are components of a similar clone ( Figure 13). “…Extensive ossification with only small remnant foci of typical BCC may eclipse diagnostic histology. BCC with monster cells 12 show scattered pleomorphic epithelial tumor cells within the tumor nodule rather than being within the stromal component. Mixed tumor of the skin can show ossification within the chondromyxoid stroma and may show follicular differentiation mimicking the BCC component.…”

“…[4][5][6][7][8] Of particular concern are those found in Spitz and ''ancient'' nevi where immunohistochemical staining may support a diagnosis of a melanocytic process. Garcia et al 5 described 15 patients with 19 basal cell carcinomas containing monster cells and termed them ''pleomorphic''.…”
