Basal Cell Carcinoma of the Penis: A Case Report and Review of the Literature

Abstract: Basal cell carcinoma of the penis is an extremely rare entity, accounting for less than 0.03% of all basal cell carcinomas. Fortunately, wide local excision of such lesions is generally curative. Fewer than 25 cases have been reported in the literature describing penile basal cell carcinoma. Here we report a case of penile basal cell carcinoma cured with wide local excision.

“…Vulvar basal cell carcinoma represents 1–5% of all vulvar cancers, 115 while penile and scrotal basal cell carcinoma is even more rare, accounting for less than 0.03% of all BCCs in men 116,117 . Dermoscopy features have been described for vulvar basal cell carcinoma only, revealing fine, reddish, well‐focused arborizing or linear vessels 18,118–120 .…”

“…Consistent with previous reports ( 24 ), in this cohort, most vulvar BCCs occurred on the labia majus, accounting for 89.6% of all vulvar BCC cases with known explicit location. BCCs of the penis were less frequently reported than BCCs of the scrotum ( 25 ), and in this cohort, the total number of penis BCCs accounted for only one-third of the scrotum BCCs. We found that penis BCCs showed poorer prognosis than scrotum BCC, with about 1.8 times higher risk of death.…”

“…When assessed according to the genital site, BCCs were found to occur far more frequently on the scrotum compared to other penile cancers. Previous reviews of the literature have reported that most penile BCCs occur on the penile shaft 10,11 . The number of reported cases is relatively small.…”

“…The BCCs in our study ranged from 8 × 8 mm to 11 × 13 mm in surface size. Although BCCs generally have a favorable prognosis due to minimal risk of spread, late presentations in this site may result in more extensive surgical management with associated physical and psychological morbidity 11 . Genital BCCs have significantly increased rates of metastasis compared to extragenital BCCs, accounting for 7% of all metastatic BCCs 12,13 .…”
