Bartter's syndrome and diabetes mellitus

Abstract: We here report a case of Bartter's syndrome occurring in association with diabetes mellitus. The patient, an insulin-dependent diabetic, presented with hypokalaemia, inappropriate kaliuresis and metabolic alkalosis. He had high plasma renin activity, relatively low plasma aldosterone, and resistance to infused angiotensin II. A high potassium diet raised total body potassium and serum potassium, did not affect plasma renin activity, but raised plasma aldosterone significantly and did not alter the resistance t… Show more

“…Example of drugs used for treating comorbid conditions like hypertension in diabetes are thiazide or loop diuretics, Angiotensin Converting Enzyme Inhibitors (ACEIs) and Angiotensin 2 Receptor Blockers (ARBs) which may also result in EI. Furthermore insulin secretion is decreased in hypokalemia 7,8 , and K+ infusions improve glucose tolerance by increasing insulin and proinsulin concentrations 9 .…”

“…The potential causal relationship between BS and type 2 diabetes mellitus remains uncertain 18. Venkat et al 19 reported that patients with BS displayed elevated insulin concentrations and hyperplasia of the islets of Langerhans was noted during autopsies and also described worsened glycaemic control following a late-onset BS diagnosis. However, it is worth noting that hypokalaemia can lead to decreased insulin secretion.…”

“…In the literature a few case reports describe idiopathic BS-like phenotype in association with type 2 diabetes. Venkat et al described patients with BS who displayed elevated insulin concentrations, with hyperplasia of the islets of Langerhans noted at autopsy [ 18 ]. Worsening of glycemic control following a diagnosis of late onset BS was described in another case report [ 19 ].…”
