Bariatric Nutrition Guidelines for the Indian Population

Abstract: It is recommended that all patients undergo compulsory pre-operative nutritional counseling and nutritional investigations and that nutritional follow-up be continued lifelong. In addition, long-term implications like hypoglycemia, dumping syndrome, sugar cravings, and weight regain, need to be picked up and managed efficiently. Most importantly, post-operative supplementation is a must irrespective of type of surgery.

“…This has resulted in a heavily carbohydrate-based diet (> 65–70% total energy intake [ 34 ]) which may be significantly lacking in adequate diversity for provision of other important nutrients [ 35 ]. Widespread lactovegetarian preferences have further reduced the scope for dietary diversity [ 36 ].…”

“…Studies across a number of low-income countries in Africa and South Asia have suggested a strong link between meat consumption in young children and lower stunting rates (Krebs et al, 2011). As India has strong lactovegetarian preferences (Remedios et al, 2016) and one of the highest rates of childhood stunting globally (FAO, 2013) this is an important consideration.…”

“…However, bariatric surgery is not entirely risk-free and carries potential complications ranging from 5 to 20% (11). Consequently, bariatric guidelines (12)(13)(14)(15) support an intentional preoperative weight loss to reduce the liver volume (13,14) and risk of perioperative complications (12,13,15). It is particularly challenging for surgeons to elevate the left lobe of a large or fatty liver (16,17), especially during laparoscopic Roux-en-y gastric bypass surgery (RYGB), thereby, increasing the risk of liver injury and intraoperative bleeding (18).…”

“…The most recent bariatric guidelines (14,15) and literature (24,(29)(30)(31)(32)(33) are in favour of energy-restricted diets to achieve rapid preoperative weight reduction. However, there is significant heterogeneity in the type of energy-restricted dietary regimes being prescribed (34).…”
