Bacteremia Caused by Arcobacter butzleri in an Immunocompromised Host

Abstract: Arcobacter butzleri is an emerging pathogen that has been implicated as the causative agent of persistent watery diarrhea. We describe a case involving a patient with chronic lymphocytic leukemia who developed invasive A. butzleri bacteremia. This case illustrates the unique challenges involved in diagnosing infections caused by emerging gastrointestinal pathogens. CASE REPORT

“…The highest prevalence is reported for poultry followed by pork and beef meat (Van Driessche and Houf, 2007). In humans, A. butzleri has been frequently associated with enteritis and occasionally bacteraemia (Arguello et al, 2015), whereas A. cryaerophilus and A. skirrowii have been isolated from stool of diarrheic patients Vandenberg et al, 2004;Wybo et al, 2004). In animals, Arcobacter is associated with reproductive disorders, mastitis and diarrhoea (Ho et al, 2006;Logan et al, 1982;van Driessche et al, 2003).…”

“…Recently, a case of A. butzleri bacteremia was reported from a patient with chronic lymphocytic leukemia (Arguello et al 2015). A. butzleri was isolated from blood of a 69-year-old woman (Lau et al 2002).…”

“…has also been attributed to one of the bacterial agents of traveler's diarrhea along with E. coli, Shigella, Salmonella and Campylobacter (McGrego & Wright 2015). Recently, an invasive form of Arcobacter infection in an 85-year-old immunocompromised man was documented (Arguello et al 2015).…”

“…A. butzleri was detected by 16S rRNA gene sequencing of the blood culture of a 69-year-old woman with acute appendicitis (Lau et al 2002), and was also isolated from the blood culture of a 60-year-old man with liver cirrhosis and a high fever (Yan et al 2000). Arguello et al (2015) described a case of an 85-year-old man with fever, hypotension, and a chronic, persistent diarrhea. In this case, A. butzleri was identified by 16S rRNA sequencing.…”
