Axillary transpleural sympathectomy: Indication, technique, and results

Abstract: The indications and operative technique for high thoracic sympathectomy by the transaxillary approach are demonstrated. The late results and complications are referred to in a retrospective analysis of 159 patients on whom 193 sympathectomies have been performed over the last 15 years for peripheral vascular occlusive disease, vasospastic diseases such as Raynaud's phenomenon, and hyperhidrosis of the upper extremity.

“…69,70 An extrapleural transaxillary approach with resection of the first rib has also been described. 71 Bilateral sympathectomy by this approach was usually performed as a staged procedure and the results reported in several series 16,[72][73][74] are similar to those of the other surgical methods.…”

“…Although various methods of sympathetic surgery have been proposed [1,5,14,16,22], the endoscopic procedure currently remains the most popular and acceptable method. The reasons derive from its simplicity, safety, and efficacy [3,9,10].…”

“…Although Linder et al [31] observed 80 % longterm improvement in patients with Raynaud's disease, other large series showed failure of sympathectomy in two thirds of patients within a year [11] and in a total of up to 81 % of cases [10]. Based on these results, sympathectomy should be reserved mainly for cases with imminent tissue loss or established ulcers.…”

“…Regarding recurrence of hyperhidrosis, all three accesses (posterior [8,17,43], supraclavicular [6,9,20,27,46], and transaxillary [12,22,31,47]) have a high long-term success rate (more than 90 %), without significant difference between the techniques. The choice of surgical approach should, therefore, be based on factors other than the success rate of the operative technique.…”
