The objective is to reveal the results of patients who underwent pars plana vitrectomy (PPV) in the same session due to lens nucleus drop during cataract extraction and to compare the refractive results according to uncomplicated phacoemulsification (phaco) surgery. Methods: The study included 26 eyes of 26 patients who underwent PPV due to lens or lens fragments drop. Preoperative and postoperative best-corrected visual acuity (BCVA), intraocular pressure, spherical equivalent, and the intraocular lens (IOL) implantation methods applied were recorded. Refractive results were compared with the spherical equivalent of 24 eyes of 19 patients who underwent uncomplicated phaco surgery. Results: Three-piece IOL was implanted in the ciliary sulcus in 20 (77%) patients, and IOL was implanted in one (4%) patient with sutureless scleral fixation using the Yamane technique. In 5 (19%) cases, the surgeries were terminated as aphakic. Preoperative BCVA was 1.3±0.5 logMAR, and postoperative BCVA was 0.29±0.4 logMAR (P<0.001 for both subgroups). Preoperative spherical equivalent was −4±2 D, and it was −0.8±1.4 D after the operation (P<0.001). In patients with PPV and IOL implantation, the postoperative spherical equivalent was −0.8±1.4 D, and it was measured as −0.7±0.6 D in the phaco-only group (P=0.37). Conclusion: It is possible to achieve optimal results in uncomplicated phaco surgery using IOL measurements calculated with appropriate biometric formulas and advanced optical biometry devices in patients who have undergone PPV due to lens nucleus drop. IOL implantation can be easily planned in the same session for patients undergoing PPV.

In order to meet the demands of myopia prevention, as well as the increasing needs of measurement for refractive and cataract operations in China, a new axial length (AL) measurement system combining an improved Twyman-Green interferometer with digital signal processing has been established. The ALs of 33 eyes of different optical refractive subjects (À8.50~0.50 D) were measured with the New AL and intraocular lens (IOL) master. The repeatability of measurements with the New AL was assessed using coefficient of variation (CoV) and intra-group correlation coefficient (ICC). Comparison, correlation, linear regression and agreement of AL between devices were analyzed. There was good repeatability (CoV = 0.0617%, ICC = 0.9999) with the New AL and great agreement has been obtained with both devices. These show that the New AL is capable of providing precise AL values over the normal AL range compared to the IOL master, and indicate that the New AL developed can be used for routine clinical AL measurements.

Axial Length in Orthokeratology Patients: Large Case Series

Purpose: To evaluate axial length measurements obtained over a three-year period in children undergoing Orthokeratology (Ortho-K).Methods: Patient records were identified from a specialty contact lens practice. Data were obtained from myopic patients who were prescribed Ortho-K wear. Demographic data at baseline included age at commencement of Ortho-K and ethnicity. Clinical data recorded at baseline and at years 1, 2 and 3 included subjective refraction, corneal topography, corneal curvature, axial length, pupil size, and lens design, changes in lens parameters, lens wear habits, wearing time and unaided visual acuity. The primary outcome measure was change in axial length during the study period.Results: 194 eyes of 97 subjects were included in data analysis. Mean age at start of Ortho-K was 10.4 years (SD=1.9). Mean axial length was 24.59 mm (SD=0.85) at baseline and 24.87 mm (SD=0.87) at the three year evaluation, for a mean change in axial length of 0.28 mm (SD=0.64). 65.5% of eyes (n=127) showed little or no change (<0.5 mm) in axial length during the study period. 20.1% of eyes (n=39) showed moderate increase in axial length of 0.5-1.0 mm and14.4% of eyes (n=28) showed >1.0 mm increase in axial length during the 3-year study period. Linear mixed regression analysis showed a significant association between older age at initiation of Ortho-K and less increase in axial length (p=0.0077). Axial length changes were not associated with duration of wear (both hours/night and nights/week) or baseline corneal curvature. Conclusion:Use of Ortho-K may help reduce axial length elongation in myopic eyes. Age of the initial treatment is a key factor. The baseline of cornea curvature and duration of the wear are not directly associated with treatment outcome. Of note, during the three-year study, 65.5% of treated eyes (n=127) showed a minimal or clinically insignificant increase of axial length (<0.5mm). Early treatment should be considered.
