Os autores avaliaram 33 pacientes (48 pés) com hálux valgo moderado a grave tratados cirurgicamente com a técnica de osteotomia de base do I metatarso em cunha de adição associada a reparação de tecidos moles. Comparações correlacionando sexo, idade e parâmetros radiográficos, foram feitas pelo teste t para amostras independentes; o desvio do sesamóide foi comparado pelo teste não paramétrico de Kruskal-Wallis. Todos os pacientes responderam a um questionário no qual expressaram o grau de satisfação com o resultado final da cirurgia. 76% dos pacientes eram do sexo feminino, com idade entre 14 e 60 anos (média de 38,15 anos), com tempo médio de seguimento de 41 meses. Em relação ao sexo não houve diferença na variação radiográfica. Também não foram constatadas diferenças no pré e pós-operatório quando comparado os valores médios dos pés direito e esquerdo. A avaliação radiológica apresentou 73% de resultados excelentes e o grau de satisfação total foi de 60,7% . Não há evidência da idade média influenciar no grau de satisfação, porém acima de 60 anos todos os pacientes relataram satisfação parcial. Concluímos que o procedimento cirúrgico apresentado seja um método seguro e eficaz para o tratamento do halux valgo moderado e grave.

Objective: To evaluate the correction of the intermetatarsal angle after arthrodesis of the metatarsophalangeal joint of the hallux. We believe that varus deformity of the first metatarsal can be corrected after arthrodesis of the first metatarsophalangeal joint, without the need for proximal osteotomy. Methods: Forty-three feet of patients who had undergone arthrodesis of the first metatarsophalangeal joint between May 1997 and October 2009 were retrospectively analyzed by means of radiographs. The mean length of follow-up was 58 months. Measurements on the metatarsophalangeal angle, intermetatarsal angle and sesamoid dislocation were made using radiographs made before, immediately after and later on after the operation. The authors declare that there was no conflict of interest in conducting this work This article is available online in Portuguese and English at the websites: www.rbo.org.br and www.scielo.br/rbort angle was 37.6 degrees preoperatively, 12.8 degrees immediately after the operation and 16.4 degrees later on after the operation. The mean intermetatarsal angle was 16 degrees preoperatively, 10 degrees immediately after the operation and 10.2 degrees later on after the operation. Regarding sesamoid dislocation, preoperative radiographs showed most feet to be classified as G3; immediately after the operation, most were classified as G2; and later on after the operation, most were G1. Conclusion: The intermetatarsal angle and sesamoid dislocation improved with arthrodesis of the first metatarsophalangeal joint, without the need for osteotomy at the base of the first metatarsal.

Resumo Objetivo Este estudo propõe a utilização da Caixa Niza, dispositivo criado com a finalidade de diminuir os erros interpretativos entre profissionais e facilitar o correto posicionamento das estruturas radiografadas ao padronizar as incidências radiográficas ortopédicas do pé anteroposterior, perfil com carga e Saltzman. Métodos Pesquisa descritiva, material coletado em Ambulatório de Ortopedia em serviço terciário de cidade brasileira de grande porte. Utilizado aparelho de radiografia marca Lotus X, modelo HF 500M, capacidade de 500 miliamperes e 125 quilovolts, distância foco-filme de 100cm, chassi radiográfico 24 × 30 cm e os comandos do aparelho ajustados para 100 mA, 5mA/seg e 60 quilovolts dependendo do tamanho variável dos pés. Exames realizados pela mesma equipe de técnicos em radiografia previamente treinados com supervisão dos autores. O chassi é posicionado em três espaços específicos da Caixa conforme a incidência proposta. Foram analisados dados de 50 imagens de pessoas entre 18 e 70 anos. Resultados A avaliação das radiografias após utilização do dispositivo proposto ocorreu de modo geral com qualidade satisfatória, permitindo correta identificação dos elementos anatômicos do pé e tornozelo e reconstrução angular. Pequenas variações nas imagens devido ao tamanho dos pés são aceitáveis e esperadas, sendo possível perceber padronização das radiografias. Conclusão A Caixa proposta se mostra um bom método de minimizar as interferências e evitar erros de interpretação radiográfica, proporcionando qualidade e agilidade ao exame, diminuindo custo e repetições desnecessárias. É inovador, um dispositivo de baixo custo, de material reciclável e biodegradável.
