AVADAR (Auto-Visual AFP Detection and Reporting): demonstration of a novel SMS-based smartphone application to improve acute flaccid paralysis (AFP) surveillance in Nigeria

Abstract: BackgroundEradication of polio requires that the acute flaccid paralysis (AFP) surveillance system is sensitive enough to detect all cases of AFP, and that such cases are promptly reported and investigated by disease surveillance personnel. When individuals, particularly community informants, are unaware of how to properly detect AFP cases or of the appropriate reporting process, they are unable to provide important feedback to the surveillance network within a country.MethodsWe tested a new SMS-based smartpho… Show more

“…However, in further analysis, it is clearly shown notification within 24 hours increased over the period in 2.4% change. This reflects the contribution of the effects of weekly feedback to countries for the improved surveillance decisions, community AFP surveillance and the use of innovative adaptive surveillance strategies such as AVADAR [ 13 , 14 ].…”

“…Furthermore, extensive environmental surveillance have been initiated around the world to supplement AFP surveillance and in the East and Southern Africa sub region where by December 2019 was already being implemented in 9 countries [ 9 , 10 ]. The use of adaptive innovative surveillance strategies such as electronic surveillance and geographical information system platforms provided surveillance information and intelligence against security threats and made polio eradication easier, more accountable, more focused/targeted and effective than in the previous years, studies evidence in Nigeria, Liberia, Papua New Guinea and other many countries to mention few [ 11 - 14 ]. The available evidence is supporting that poliomyelitis will remain to be the second disease to be eradicated globally even though the pathway had more programmatic and strategic program environment huddles than it was for smallpox eradication.…”

“…The technology supported the monitoring and tracking of the movement of immunisation teams and the estimation of immunisation coverage, and included electronic surveillance; integrated supportive supervision, which facilitated active surveillance in health facilities in real time; and AVADAR (auto-visual acute flaccid paralysis detection and reporting)—an SMS-based smartphone application used by community informants in areas with weak health systems. 16 Polio surveillance reviews and retrospective searches for missed cases of acute flaccid paralysis were done and enhanced environmental surveillance for polioviruses was implemented. 5 Regular and rigorous supervision, evaluations, and reviews focusing on surveillance performance at subnational levels were also implemented.…”

“…In addition, performance indicators for AFP surveillance require that reporting be timely and complete, and represent the geography of the country. [2]. All cases of AFP should be investigated and two stool samples should be collected from each AFP case at least 24 hours apart for viral isolation in a WHO-accredited laboratory [2].…”

“…[2]. All cases of AFP should be investigated and two stool samples should be collected from each AFP case at least 24 hours apart for viral isolation in a WHO-accredited laboratory [2]. The quality of AFP surveillance is therefore critical in countries moving toward the final phases of polio eradication [3].…”
