Automatic aortic anastomosis with an innovative computer-controlled circular stapler for surgical treatment of aortic aneurysm

Abstract: The computer-controlled stapling device enabled reliable anastomosis of the aorta and the artificial graft. This study showed that mechanical vascular anastomosis with the iDrive was sufficiently strong and safe relative to manual suturing.

“…Previously we conducted experimental studies on the use of automatic anastomotic devices for anastomosing the aorta to a vascular prosthesis. 4,5 The case presented here is the first clinical application of such a device to aortic anastomosis. Use of an automatic anastomotic device in aortic reconstruction for thoracic aortic aneurysm in this patient with low left ventricular function greatly shortened the circulatory and cardiac arrest time and proved to be safe and effective.…”

“…[5][6][7] Some coronary bypass anastomotic devices have been commercially available. 10,11 Takata and colleagues 11 recently tested a computer-controlled circular stapler device (SurgAssist iDrive; Power Medical Interventions, Dublin, Ireland) that performs anastomosis of the aorta and an artificial graft. Similarly, Lachat and colleagues 12 have used covered stent devices bridged across the target vessel and graft to perform quicker medium vessel anastomoses during hybrid thoracoabdominal repair.…”

“…In fact, the architecture of the connector acting as a stent was designed to preserve the vascularization of the arterial wall with a helical arrangement of spikes, which is different from other systems that crush the arterial wall or staple the wall circumferentially. 11,[19][20][21] Finally for the in vivo procedures, because of the risk of aortic thrombosis when clamping the aorta in pigs, 15 we used a large dose of clopidogrel in the days before the procedure. This was specifically related to the pig model and should not be translated to the administration of a high dose of anticoagulants and antiplatelet drugs when used in humans.…”
