Autoimmune hemolytic anemia associated with ulcerative colitis

Abstract: Three patients are reported in whom autoimmune hemolytic anemia developed during the course of ulcerative colitis. A review of the literature yielded 11 additional cases, and the clinical, immunologic, and therapeutic characteristics are summarized. The results of steroid therapy and splenectomy are similar to those for idiopathic autoimmune hemolytic anemia. The cause and effect relationship between these two diseases is not clear, but colectomy appears to produce remission in hemolysis when the latter is ref… Show more

“…Red cell autoantibodies associated with pregnancy are rare and the present series, to the best of our knowledge, is the largest cases), which has a well-recognised associa tion with the development of erythrocyte autoantibodies and AIH [24,25]. Unfortu nately, these 4 patients had not had recent serological testing prior to pregnancy and so it was not possible to determine whether the routine antenatal testing merely detected pre-existing serological abnormalities.…”

“…Autoimmune hemolytic anemia seems to be a rare complication of UC; Ormerod (9) described one case in 479 patients and Edwards & Truelove (10) recorded one case in 624 patients. A positive direct Coombs' test is expected to appear more frequently than autoimmune hemolytic anemia, since antibody coating of red cells may appear without laboratory findings indicative of hemolysis (3,11). This presumption is supported in our study, which reveals a prevalence of 1.82 % (95 % confidence limits 0.2-6.3 %).…”
