Auto-transfusion tourniquets: the next evolution of tourniquets

Abstract: In this article, we discuss the relationship between hemorrhagic shock and the pathophysiology of shock using conventional tourniquets. We will focus on corollary benefits with the use of HemaClear®, a self-contained, sterile, exsanguinating auto-transfusion tourniquet. This discussion will demonstrate that the use of auto-transfusion tourniquets is a practical evidence-based approach in fluid resuscitation: it shortens the duration of shock after hemorrhage and trauma compared with conventional tourniquets. “…A particular form of intraoperative autotransfusion is used in emergency medicine for cases involving signifi cant hemorrhage. In such cases, the extremities are used as reservoirs of exactly matched blood (500 ml blood/adult leg), which can be temporarily tapped in order to keep the patient stable for a limited time [ 8 ] . In our case, suspending the tumor mass during the excision did facilitate the autotransfusion.…”

“…Bei solchen Fällen werden die Extremitäten als Speicher von genau angepasstem Blut (500 ml Blut/erwachsenes Bein) verwendet. Diese können vorübergehend angezapft werden, um den Patienten für eine begrenzte Zeit stabil zu halten . In unserem Fall erleichterte das Suspendieren der Tumormasse während der Exzision die Eigenbluttransfusion.…”
