Audiological Alterations in Patients With Cleft Palate

Abstract: Hearing improvement prior to language acquisition is essential for a proper speech development. Early trans tympanic tubes implantation during cleft palate repair contributes to a correct short-term ventilation of the middle ear, being the T tubes the best option.

“…TTS has shown itself to be an effective tool for treating middle ear effusion in cleft patients. 9,24 Some authors suggest TTS to be performed concomitant to palatoplasty, 9,14,25 as it can improve hearing development in the patient. Even early long-term tympanostomy tubes prior to palatoplasty are considered a treatment option for increasing middle ear ventilation.…”

“…13 In current literature, different approaches are presented for the treatment of patients with cleft palate and with conductive hearing loss due to effusion in the tympanic cavity. [14][15][16][17] Paracentesis and tympanostomy tube surgery (TTS) are the main treatment options for correcting middle ear dysfunction. This study evaluates the prevalence of tympanostomy tube placement in patients with cleft deformities in relation to the absolute and relative cleft width and cleft classification.…”

“…Hypoacusis is a frequent comorbidity in patients with CL/P, especially in those with palatal involvement. Muscular alterations of the soft palate, which produce Eustachian tube dysfunctions, can derive in hearing loss ( Rivelli et al, 2018 ). Hypoacusis in our study population was the third most frequent clinical feature associated with CL/P, with 1.88% of patients.…”

“…5,6 In children with cleft palate, OME is typically more severe and more persistent. 17 This is thought to be primarily due to impaired formation and function of the eustachian tube, 18 as well as abnormal insertion of tensor veli palatini and levator veli palatini muscles. 19 These structures normally assist with clearance and drainage of middle ear fluid.…”
