Atypical presentation of lipomatous hypertrophy of the interatrial septum: a case report

Abstract: Background Lipomatous hypertrophy of the interatrial septum (LHIAS) is a common finding on transthoracic echocardiogram (TTE). Occasionally, the appearance of LHIAS is atypical and multimodality imaging is helpful to make the diagnosis. We present a case of atypical LHIAS to highlight the potential aetiologies for an interatrial septal mass and review features on multimodality imaging that help decrease uncertainty and establish a diagnosis. “…Thus, in a series that included 11 patients with LHIS examined with 18 FDG PET-CT, 9 (82%) of these patients showed focal increased FDG uptake corresponding to the region of LHIS on cross-sectional imaging, with a mean standard uptake value of the atrial septa of 5. 4 As reported in our case, the increased uptake of 18 FDG PET-CT in LHIS could be misdiagnosed as an uncontrolled infection (abscess).…”

“…However, it is not an uncommon finding on transthoracic echocardiograms and can be similar to other cardiologic processes. 4 A complete review of LHIS describes a complete differential diagnosis and complications of LHIS. This entity could be similar to some neoplasm.…”

“…Lipomatous hypertrophy of the interatrial septum typically spares the fossa ovalis while respecting the atrial septum boundaries 6 . TTE has low sensitivity in diagnosing LHIAS due to limited resolution, especially when the image quality is suboptimal.…”

“…Lipomatous hypertrophy of the interatrial septum typically spares the fossa ovalis while respecting the atrial septum boundaries. 6 TTE has low sensitivity in diagnosing LHIAS due to limited resolution, especially when the image quality is suboptimal. A transesophageal echocardiogram can be used to easily diagnose LHIAS, which best appears "in the bicaval view" as "globular thickening of the interatrial septum.…”
