Atrial septal aneurysm associated with additional cardiovascular comorbidities in two middle age female patients with ECG signs of right bundle branch block: two case reports

Abstract: Introduction: Atrial septal aneurysm (ASA) is often associated with other atrial septal abnormalities, particularly with atrial septal defect type ostium secundum or patent foramen ovale. ECG signs of incomplete or complete right bundle branch block are known to be associated with atrial septal defects, however such correlation with other atrial septal abnormalities is not documented.

“…21 We presented earlier two cases of middle-aged women diagnosed for the first time with atrial septal aneurysm associated with additional cardiac anomalies -such as patent foramen ovale, mitral valve prolapse, atrial septal defect type ostium primum, and atrial septal defect type ostium secundum -that presented with signs of right bundle branch block on electrocardiogram. 22 We highlighted the importance of transoesophageal echocardiography as a method to diagnose interatrial septal abnormalities, as well as the value of right bundle branch block as a possible clue that may lead ASD ASA MR MS AR VSD TVP MVP ASD 1 2 4 1 1 1 1 ASA 5 1 1 PFO 6 12 6 1 2 Total 1 8 21 1 8 1 2 3 AR 5 aortic regurgitation; ASA 5 atrial septal aneurysm; ASD 5 atrial septal defect; MR 5 mitral regurgitation; MS 5 mitral stenosis; MVP 5 mitral valve prolapsed; PFO 5 patent foramen ovale; TVP 5 tricuspid valve prolapse; VSD 5 ventricular septal defect to early diagnosis of these disorders. 22 These cases encouraged us to conduct the current study in order to evaluate the relationship of atrial septal abnormalities with right bundle branch block in a larger group of patients.…”

“…ASA is well identified by echocardiography. TEE is more sensitive than TTE to diagnose ASA because the interatrial septum is visualized more clearly . In this case, the interatrial septum showed a rare “aneurysm in aneurysm” structure.…”

“…There are many evidences associating Atrial Septal Aneurysm (ASA) and cardiac impulse conduction disorder 6,7 . Several reports have shown possible association of Right Bundle Branch Block (RBBB) and ASA 6,7 but few or no data in medical literature have reported association between ASA and Congenital Conducting defect (CD).…”
