Athlete’s Hepatitis in a Young Healthy Marathon Runner

Abstract: Athlete’s hepatitis is an uncommon clinical condition characterized by a significant ischemic insult to the liver. The most likely pathogenesis is decreased blood supply to the liver resulting in significant hypoxic hepatocyte injury. We report a case of a 48-year-old healthy young male who presented with athlete’s hepatitis confirmed by liver biopsy after a 14-mile marathon run. To our knowledge, this is the second case of athlete’s ischemic hepatitis reported in the literature. It is a diagnosis worth consid… Show more

“…On laboratory workup, viral infections such as hepatitis (A, B, C), Epstein-Barr virus, cytomegalovirus, herpes simplex virus, HIV, autoimmune, and metabolic causes were ruled out. Since his blood pressure was stable until day 8 of his hospitalization without any pressor support, ischemic hepatitis was unlikely [ 20 ]. Tylenol toxicity was excluded (levels were less than 5 ug/mL).…”

“…These processes are similar to those found during and after successful recovery from tissue damage due to insult, such as myocardial infarct 34 . For example, some athletes involved in extreme exercise programs may experience either “athlete’s hepatitis” 35 or cardiac myopathies as a result of either overuse or repeated transient ischemia 36 . We propose that the immune system is modified in a novel way during migration to help remove and repair the physical damage to these tissues caused by overexertion during long distance flight.…”
