Infectious agents have been proposed as one of the risk factors for schizophrenia. However, the data on the association of infectious agents with in vivo brain changes are scant. We evaluated the association of serological evidence of exposure to herpes simplex virus 1 (HSV1) with in vivo brain structural variations among first-episode antipsychotic-naive schizophrenia/ schizoaffective disorder patients and control subjects. We assayed HSV1 immunoglobulin G (IgG) antibody in serum samples from 30 patients and 44 healthy subjects and obtained structural magnetic resonance imaging scans from the same individuals. There were proportionately more patients with elevated HSV1 antibody ratios than healthy comparison subjects (v 2 = 3.98, 1 df, P = 0.046) and patients had significantly higher HSV1 IgG antibody ratios compared with healthy subjects. Using optimized voxel-based morphometry, we examined diagnosis by HSV1 serological status interaction followed by within-and betweengroup comparison across the serological status. We observed a diagnosis by HSV1 serological status interaction and a significant main effect of HSV1 serological status in the prefrontal gray matter. Patients exposed to HSV1 had decreased gray matter in Brodmann area 9 (dorsolateral prefrontal cortex) and 32 (anterior cingulate cortex) compared with patients without serological evidence of exposure to HSV1. HSV1-associated differences in brain structure were not detected among healthy subjects. These findings suggest that HSV1 exposure in schizophrenia is associated with specific regional gray matter differences that may not be attributable to medications, illness chronicity or comorbid substance use. This study provides suggestive evidence for a link between HSV1 exposure and some of the cerebral morphological changes often reported in schizophrenia.

Considering the results of epidemiological studies, claimed that some congenital and postpartum infections in the intrauterine period, especially in 2nd Trimester, may play a role for the occurrence of Schizophrenia and Bipolar Disorder (BD). Aim of our study; serum samples of schizophreni-BD patients and healthy asymptomatic donors, Toxoplasma gondii (T.gondii), Chlamydia pneumoni (C.pneumoniae), Chlamydia trachomatis (C.trachomatis), Cytomegalovirus (CMV), Herpes simplex Virus(HSV1-HSV2), Treponema pallidum (T.pallidum) and Barnavirus which associated with psychic disorders evaluated using serological diagnostic methods and investigated the possible relationship of infections. Material and Methods: In this study, ELISA method was used for seroprevalence of congenital and postpartum infections. Also, Sandwich EIA method was used for Endogenous Bornavirus Like Nucleoprotein1 (EBLN1) in peripheral blood samples. Results: Serum samples from 258 patients consisting of 96 female-162 male observed in psychiatry clinics and from healthy asymptomatic 150 donors including 100 male-50 female who came to the blood centers to donate blood. A significant difference wasn't detected among seropositivity rates of CMV, HSV1-HSV2 and T.pallidum for patients and control groups. In statistical aspects, seroprevalence of T.gondii and C.pneumoniae in patients (93.02%-68.99%) were found significantly higher than the seropositivity rate of the control groups (63.33%-50%). Despite C.trachomatis showed a statistically significant difference between patients and control group of women compared to patients and control group of men, but significant difference weren't detected in whole investigated population. BVLN1 that belong Bornavirus was detected 4 men (2.46%) in the patient group. Conclusion: Based on study results, T.gondii and C.pneumoniae carry higher risk in patients with schizophrenia and BD compared to control ones.

We investigated the possible involvement of OTX2, a homeobox gene crucial for forebrain development, in the pathogenesis of schizophrenia and bipolar disorder. The disruption of this gene results in cortical malformations and causes serotonergic and dopaminergic cells in the midbrain to be expressed in aberrant locations. Resequencing of DNA from OTX2 exons and surrounding introns from 60 individuals (15 schizophrenia, 15 bipolar disorder, 15 depression, and 15 control) revealed two intronic polymorphisms, rs2277499 (C/T) and rs28757218 (G/T), but no other variations. The minor allele of rs2277499 (T) did not associate with clinical diagnosis. However, using a Taqman genotyping assay, we found the rs28757218 minor allele (T) in 30 out of 720 (4.2%) individuals with bipolar disorder but only in 6 out of 526 (1.1%) control individuals (odds ratio 3.5, 95% confidence interval 1.4-10.4, P = 0.003). On the other hand, the rs28757218 minor allele was only found in 6 out of 458 (1.3%) individuals with schizophrenia. All individuals with the rs28757218 polymorphism were heterozygous for the allele. Based on this positive case-control association finding, we conclude that variations in OTX2 might confer risk for the development of bipolar disorder.
