Association of free vitamin D3 concentrations and asthma treatment failures in the VIDA Trial

Abstract: The use of free concentrations was inferior to total concentrations as a biomarker of efficacy of vitamin D supplementation in VIDA trial participants. Future studies of vitamin D status in patients with asthma should measure both free and total concentrations to better understand which marker of vitamin D function is most informative.

“…In addition, our findings suggest that VDBP-levels do not correlate with the levels of free 25-OHD in the circulation. This is in contrast to some previous reports also using direct methods to measure the free fraction [ 11 13 ]. However, taking into account that less than 5% of the binding sites on VDBP are occupied by 25-OHD [ 3 ], it is reasonable to suggest that VDBP levels only have a minor impact on the levels of free 25-OHD.…”

“…Some study limitations occurred. Like many other observational studies, [52][53][54] this was an ancillary analysis of data from a randomized clinical trial designed to investigate a different research question, which may limit generalizability of our results. FFQ analysis of nutrient intake is subject to presumably non-differential measurement bias.…”

“…In some studies free serum 25(OH)D levels were inversely associated with coronary artery disease (125), pediatric inflammatory bowel disease (126) and ulcerative colitis (127), insulin sensitivity (128), reduction in lipid markers in statin patients (129), and acromegaly (130). However, in some other studies free 25(OH)D was inferior in asthma (131) and no better than total 25(OH)D for colorectal cancer in African-Americans (132).…”
