Association Between Visit-to-Visit Blood Pressure Variability in Early Adulthood and Myocardial Structure and Function in Later Life

Abstract: IMPORTANCE Long-term blood pressure (BP) variability has emerged as a reproducible measure that is associated with heart failure independent of systemic BP. Visit-to-visit BP variability may be associated with the risk of heart failure early in the life course and thus may be reflected in subclinical alterations in cardiac structure and function.OBJECTIVE To evaluate the association between visit-to-visit BP variability in early adulthood and myocardial structure and function in middle age. DESIGN, SETTING, AN… Show more

“…Prior studies document an association of higher BP variability with CVD, 7 , 8 , 9 stroke, 5 , 6 , 7 left ventricular remodeling, 36 and death. 21 , 28 , 37 , 38 , 39 , 40 , 41 However, most of these studies have focused on relatively short-term outcomes, populations without a prior history of CAD, and relatively younger populations.…”

“…For example, VVV was found to be associated with markers of cardiac, vascular, and cerebral diseases as well as albuminuria [ 8 ]. Furthermore, VVV has been related with central sympathetic overactivity, variations in renin–angiotensin–aldosterone system stimulation, adverse alterations in cardiac structure or function (i.e., left ventricular dysfunction, atrial fibrillation), as well as vascular target organ damage (i.e., carotid atherosclerosis and stiffness) in adults [ 12 , 33 ] and the elderly [ 34 ]. In pediatrics with renal disease, increased VVV was tied with reduced eGFR [ 32 ].…”

“…Last but not least, BP variability may be independently involved in the progression and severity of cardiac, cerebral and renal target-organ damage. For example, greater BPV appears to increase the risk of adverse alterations in cardiac structure in both adults and pediatrics independent of mean BP levels [ 12 , 16 ]. However, such information is not available for the current study, preventing us to effectively detect the independent prognostic value of VVV.…”

“…The CV was calculated as (SD/mean) × 100% [24]. The ARV calculated as the average absolute difference between successive measurements [25]. The VIM was calculated as 100 × SD/mean beta , where beta is the regression coefficient based on a natural logarithm of SD on the natural logarithm of the mean [26].…”
