Globally, a decreasing incidence of male esophageal squamous cell carcinoma (ESCC) has been observed in recent decades. We evaluated the determinants of esophageal distinct iodine‐unstained lesions (DIULs), high‐cancer‐risk lesions and ESCC, among 3858 Japanese alcohol‐dependent men (40‐79 years) who underwent chromoendoscopic screening between 2003 and 2018. The initial screening detected DIULs ≥ 5 mm in 541 patients (dysplasia in 319 and SCC in 129) and multiple DIULs in 640. The detection rates for DIULs and chronic atrophic gastritis (CAG), pack‐years, and the mean corpuscular volume (MCV) decreased over the course of the study period, while the detection of hiatal hernia and/or columnar‐lined esophagus (HH/CLE) and the carriers of inactive heterozygous aldehyde dehydrogenase‐2 (ALDH2, rs671) increased. Multiple logistic regression analyses showed that an older age, larger number of pack‐years, smaller body mass index, larger MCV, presence of a slow‐metabolizing alcohol dehydrogenase‐1B genotype (rs1229984), presence of an inactive heterozygous ALDH2 genotype, and more advanced degree of CAG increased the odds ratios (ORs) for DIULs, while the 2008‐2013 and 2014‐2018 screening periods had lower ORs for DIULs than the 2003‐2007 screening period. The presence of HH/CLE decreased the OR for multiple DIULs and was associated with a more proximal location of ESCC. In conclusion, the detection of DIULs in an alcohol‐dependent population decreased between 2003 and 2018. In addition to reported determinants of ESCC, CAG and HH/CLE were associated with the risk of DIULs. Enigmatically, however, the decline in the detection of DIULs was not adequately explained by these factors and warrants further research.

Objective. The purpose was to analyze the effect of early enteral nutrition (EEN) support combined with chemotherapy on related complications and immune function in patients after radical gastrectomy. Methods. 80 patients with gastric cancer treated in our hospital from March 2019 to March 2020 were selected as the research objects and divided into the experimental group and control group according to the random number table, with 40 cases in each group. The control group received chemotherapy only after surgery, while the experimental group received EEN on this basis. The total protein (TP), transferrin (TF), albumin (ALB), immune cells, and other indexes were measured in the two groups before and after treatment to analyze the effect of different treatment methods on the complications and immune function of patients after radical gastrectomy. Results. There were no significant differences in gender ratio, average age, average BMI, pathological types, disease staging, and residence between the two groups ( P > 0.05). The exhaust recovery time, total gastric tube drainage, fluid intake time, and hospitalization time in the experimental group were significantly lower than those in the control group ( P < 0.05). There were no significant differences in the TP, TF, and ALB levels between the two groups before treatment ( P > 0.05), and the TP, TF, and ALB levels in the experimental group were significantly higher than those in the control group after treatment ( P < 0.05). The CD4+/CD8+, CD3+, and CD4+ levels in the experimental group after treatment were significantly higher than those in the control group ( P < 0.001). After treatment, the growth hormone levels in both groups significantly increased ( P < 0.001), and the growth hormone level in the experimental group was significantly higher than that in the control group ( P < 0.001). There was no significant difference in the KPS scores between the two groups before treatment ( P > 0.05), and the KPS score in the experimental group was significantly higher than that in the control group after treatment ( P < 0.001). The incidence of postoperative complications in the experimental group was significantly lower than that in the control group ( P < 0.05). Conclusion. EEN combined with chemotherapy is a reliable method to improve the immune function of patients after radical gastrectomy for gastric cancer, which plays an important role in improving the physical state of patients and reducing the incidence of complications. Therefore, its further research will help to establish a better treatment plan for such patients.

Feasibility of ultrathin endoscope for esophageal endoscopic submucosal dissection

Background and study aims Endoscopic submucosal dissection (ESD) is widely performed for superficial esophageal cancer, but stricture after extensive resection is a major clinical problem. Using an ultrathin endoscope would enable endoscopists to approach lesions beyond the stricture. We evaluated the feasibility of an ultrathin endoscope for esophageal ESD. Methods To perform ESD with an ultrathin endoscope, we developed a transparent hood and ESD knife. A total of 24 esophageal ESDs were performed by two endoscopists with excised and live porcine esophaguses. A GIF-Q260 J and Dual knife were used in the conventional group and the GIF-XP260NS and a newly developed knife were used in the ultrathin group. En bloc resection rates, perforation rates, and procedure times were compared. Results All 24 lesions were resected en bloc without perforation. The mean procedure time was longer in the ultrathin group, although not significantly so (274.3 ± 81.8 s vs 435.8 ± 313.9 s, respectively; P = 0.22). Conclusion Although the procedure time was longer in the ultrathin group, en bloc resection was performed without any perforation. The findings indicate that esophageal ESD with an ultrathin endoscope is feasible.
